Serves adult men & women with the primary diagnosis of Intellectual Disability (diagnosis must be documented as originating prior to age 18). Provide care, assistance and supervision to individuals in their own homes. These services are supplemental to the care provided by a primary caregiver or they may support an individual whose level of independence does not require a primary care provider.
