Hospice and Palliative Care benefits are available to persons who have been determined to be in the end stages of a terminal illness and are no longer seeking or receiving curative treatment of any kind. The goal of hospice and palliative care is to assist in providing "in home" management of physical and emotional problems associated with terminal illness such as the relief of pain and distressing symptoms. Services are directed toward maintaining a high quality of life for both patient and family/significant other using team support and guidance.

Hospice is a large program encompassing a wide range of services and care for those afflicted with a terminal disease and their families. Hospice strives to treat the physical, emotional and spiritual needs of the patient. A nurse makes home visits to help manage symptoms, while home health aides help with personal care and companionship. Social workers aide with emotional support and advanced directives, speaking with families to determine strengths and needs, coordinate problem solving, counselling and referrals. Finally, chaplain services provide spiritual support upon request. Palliative care exists to enhance the comfort and quality of life, with no specific therapy being excluded from consideration.
Assisted living and nursing facility provides continuum of care including independent living, assisted living, short term rehabilitation, long term care, Alzheimer's and dementia care, hospice care, and intravenous care.
Physicians recommend hospice based on specific criteria. Hospice may be appropriate for patients of any age, including infants and children, who: * face terminal illness * have a life expectancy of six months or less, if the illness runs its usual course * wish to focus on quality of life * want interdisciplinary care of the patient and family who is facing a chronic or terminal illness with a focus primarily on comfort and support (palliative care) * do not want aggressive, curative therapy

Besides regularly scheduled home visits, by calling the helpline you can speak by phone to a nurse who is able to answer questions and assess unexpected situations. * Medications related to the hospice diagnosis and used for pain and symptom management are provided. * Medical equipment such as wheelchairs, walkers, hospital bed are provided as needed. * Home Care Aides provide personal care and light housekeeping for the patient. * Medical social workers assess the psychosocial needs of the patient/family. They provide education and support regarding interpersonal relationships, and are experts in community resources. This allows them to facilitate family communication and recommend resources for financial, legal and caregiving issues. * Chaplain provides spiritual support to patients and families of all faiths. * Support therapies Physical, Occupational and Speech Therapy and dietary counseling provide expert services as needed. * Pharmacist provide expertise in pain and symptom management. * Volunteers are a specially trained group of people who are available to provide supportive services to patients, families and hospice staff. * Bereavement services provide grief follow up according to the needs of the families. This includes phone contacts, supportive literature, mailings and an invitation to the annual memorial service. Grief support groups and individual counseling are available through Sentara Grief Support Services. No person will be denied Sentara Hospice services on the basis of race, color, national origin, handicap, age or inability to pay.

Provides hospice care to patients with a chronic or terminal illness residing at home or in assisted living or nursing facilities. Care is provided by registered nurses, medical social workers, nursing assistants, chaplains, bereavement counselors, dietitian and complementary therapists. Emphasizes maintaining patients in their own homes. In-patient care and residential care is available when the level of care required mitigates against home management or when the client does not have a caregiver or family member able to care for them at home. Whenever possible, the aim of in-patient care is to enable the patient and family to return to home care. Terminal illness affects every member of the family. Hospice care includes both patient and family as the primary unit of care. Family members are supported in their efforts to care for the patient themselves and to prepare for the impending death. In addition, the bereavement program and support group helps family members come to terms with their grief following the patient's death.
