Emphasizes maintaining patients in their own homes. In-patient care and residential care is available when the level of care required mitigates against home management or when the client does not have a caregiver or family member able to care for them at home. Whenever possible, the aim of in-patient care is to enable the patient and family to return to home care. Terminal illness affects every member of the family. Hospice care includes both patient and family as the primary unit of care. Family members are supported in their efforts to care for the patient themselves and to prepare for the impending death. In addition, the bereavement program and support group helps family members come to terms with their grief following the patient's death.
