An effective self-management education program for people with chronic health problems. CDSMP workshops meet 2 hours per week for 6 weeks. Workshops are facilitated by two trained leaders. CDSMP deals with problems associated with chronic disease, appropriate exercise, proper use of medications, communicating effectively with family, friends, and health professionals, nutrition, and, how to evaluate new treatments.

Direct cancer patients, survivors and their networks of support to information about cancer diagnoses, treatment options, diet and stress management; help with financial or transportation assistance and support services from peer-to-peer networks to hospice; Host events throughout the area, recruiting both nationally known speakers and local medical professionals to raise awareness about ways people can reduce the risks of getting cancer via healthy life choices and following the cancer screening guidelines.

Its purpose is to increase access to education, reduce medical costs and prevent complications for women with gestational diabetes. Initially, each participant meets with a diabetes educator for a gestational class, sets personal goals and receives follow up education in the OB Clinic.

Chronic Disease Self-Management Education (CDSME) provides workshops designed to empower adults to better manage their chronic health conditions with the goal of giving them the confidence and skills to actively manage their health care and reduce unnecessary visits to the Emergency Department or to the physician(s). Programs are offered throughout the year in various locations in the New River Valley. Each program meets weekly and typically lasts six weeks.

Centra's Diabetes program is designed to promote diabetes self-management. We provide diabetes education to patients, family members and caretakers, and we offer inpatient as well as outpatient education. All we ask is that patients provide a physician referral so we'll have information about their needs, and their medical providers will know the proper instructions. Through our program, people recently diagnosed with diabetes learn survival skills including: A basic intro to the disease process Foot care Eye and dental health Meal planning and food label reading Guidelines for sick day management Medication information (oral or insulin) Self-monitoring guidelines Instructions on use of the glucose meter Exercise to promote better control of the disease and improve well-being Managing complications Traveling skills Intensive insulin management Hotline available to answer diabetic related questions. The program offers workshops for older adults and adults with disabilities to address the challenges of living with any chronic health condition. Workshops are conducted once a week for six weeks in community settings. Workshop participants develop the skills and coping strategies they need to manage their symptoms through a group process that includes action planning, interactive learning, behavior modeling, problem solving, decision-making and most important, social support for change. Some workshops may be held for those who have specific health conidtions such as dementia or diabetes.
