Chronic Disease Self-Management Program - Richmond, Span Center, The

An effective self-management education program for people with chronic health problems. CDSMP workshops meet 2 hours per week for 6 weeks. Workshops are facilitated by two trained leaders. CDSMP deals with problems associated with chronic disease, appropriate exercise, proper use of medications, communicating effectively with family, friends, and health professionals, nutrition, and, how to evaluate new treatments.

Partner with state universities to bring research studies to those living in our communities. Direct cancer patients, survivors and their networks of support to information about cancer diagnoses, treatment options, diet and stress management; help with financial or transportation assistance and support services from peer-to-peer networks to hospice; Host events throughout the area, recruiting both nationally known speakers and local medical professionals to raise awareness about ways people can reduce the risks of getting cancer via healthy life choices and following the cancer screening guidelines.

Health Education - Pulaski, Chronic Disease Self-Management Education, New River Valley Agency on Aging

Chronic Disease Self-Management Education (CDSME) provides workshops designed to empower adults to better manage their chronic health conditions with the goal of giving them the confidence and skills to actively manage their health care and reduce unnecessary visits to the Emergency Department or to the physician(s). Programs are offered throughout the year in various locations in the New River Valley. Each program meets weekly and typically lasts six weeks.

Provides free access to HIV and Hepatitis C prevention information/education, HIV Pre-Exposure Prophylaxis (PrEP) and non-occupational Post-Exposure Prophylaxis (nPEP) counseling. Service Navigation for linkage to STI testing, PrEP and nPEP navigation, basic needs (i.e., food, housing, etc.), insurance enrollment, etc., available to anyone regardless of HIV or HCV status.

Provides patient navigation, travel assistance for medical appointments, liquid nutrition, pharmacy assistance and other financial help. Offers CancerHelp, a user-friendly database which contains the most up-to-date information about all types of cancer, cancer prevention, cancer screenings, cancer treatments and cancer research. Expands community awareness of and education about cancer issues, and enhances the quality of life of cancer patients, their family members and caregivers by addressing their psychological, social, emotional, spiritual and physical needs related to the impact of a diagnosis of cancer. Offers evidence-informed Understanding Cancer workshops through a partnership with University of Virginia Cancer Center.

Provides a program designed in accordance with the American Diabetes Association clinical practice recommendations. Its purpose is to increase access to education, reduce medical costs and prevent complications for women with gestational diabetes. Initially, each participant meets with a diabetes educator for a gestational class, sets personal goals and receives follow up education in the OB Clinic.

Centra's Diabetes program is designed to promote diabetes self-management. We provide diabetes education to patients, family members and caretakers, and we offer inpatient as well as outpatient education. All we ask is that patients provide a physician referral so we'll have information about their needs, and their medical providers will know the proper instructions.
