An effective self-management education program for people with chronic health problems. CDSMP workshops meet 2 hours per week for 6 weeks. Workshops are facilitated by two trained leaders. Centra's Diabetes program is designed to promote diabetes self-management. We provide diabetes education to patients, family members and caretakers, and we offer inpatient as well as outpatient education. All we ask is that patients provide a physician referral so we'll have information about their needs, and their medical providers will know the proper instructions. Through our program, people recently diagnosed with diabetes learn survival skills including: A basic intro to the disease process Foot care Eye and dental health Meal planning and food label reading Guidelines for sick day management Medication information (oral or insulin) Self-monitoring guidelines Instructions on use of the glucose meter Exercise to promote better control of the disease and improve well-being Managing complications Traveling skills Intensive insulin management Hotline available to answer diabetic related questions. Offers CancerHelp, a user-friendly database which contains the most up-to-date information about all types of cancer, cancer prevention, cancer screenings, cancer treatments and cancer research. Expands community awareness of and education about cancer issues, and enhances the quality of life of cancer patients, their family members and caregivers by addressing their psychological, social, emotional, spiritual and physical needs related to the impact of a diagnosis of cancer. Offers evidence-informed Understanding Cancer workshops through a partnership with University of Virginia Cancer Center.
