A self-management education program for people with chronic health problems. CDSMP workshops meet 2.5 hours per week for 6 weeks. CDSMP deals with problems associated with chronic disease, appropriate exercise, proper use of medications, communicating effectively with family, friends, and health professionals, nutrition, and, how to evaluate new treatments.
