Info Lines/Specialized I&R - Big Stone Gap, Mountain Empire Older Citizens

The first step in MEOC's Care Coordination and Options Counseling services. Handles all referrals for assistance made to MEOC and provides information about and referrals to other service providers. Receives referrals for assistance from many sources including older persons themselves, family members, neighbors, ministers, hospitals, home health agencies and others. Determines the needs of individual older persons and then links the person in need with the appropriate service(s). Visits the person, gathers information and assists in linking the person and the family with the services needed. MEOC staff are familiar with all community services, not just MEOC services, and link individuals to all services needed and available within the community.
