For routine Intakes, individuals are encouraged to call the main telephone number and request an Intake and Assessment.

This approach is long term and indefinite, with efforts prioritized to those at risk of rehospitalization. Case management services include hospital liaison and discharge planning. It provides training and education to assist individuals as they work towards stability, independent living, and bettering their daily lives.
