This approach is long term and indefinite, with efforts prioritized to those at risk of rehospitalization. Case management services include hospital liaison and discharge planning. Mental Health Skill-Building: Assists those with mental health diagnoses in meeting all of life's struggles and challenges - from helping the client to access community resources to training the client in the most basic life skills. Provides mental health supports meeting a wide range of needs associated directly with the mental illness itself and also with the accompanying issues that go along with every mental health diagnosis. SA/MH Outpatient Outpatient services consist of individual, group, and family counseling depending on the individuals need.
