This approach is long term and indefinite, with efforts prioritized to those at risk of rehospitalization. Case management services include hospital liaison and discharge planning. Mental Health Skill-Building: Assists those with mental health diagnoses in meeting all of life's struggles and challenges - from helping the client to access community resources to training the client in the most basic life skills. Provides mental health supports meeting a wide range of needs associated directly with the mental illness itself and also with the accompanying issues that go along with every mental health diagnosis. SA/MH Outpatient Outpatient services consist of individual, group, and family counseling depending on the individuals need. Services will be offered at minimum an hour each week and treatment will cover topics such as coping skills, mental health wellness, anger management, medication assisted treatment (MAT), and other concerns persons with substance abuse and/or mental health issues may face.
