For those with Medicaid, two risk factors must be present: 1)At risk of hospitalization, homelessness, or isolation form social supports due to difficulties in maintaining relationships2)Struggles with daily living activities such as food preparation, proper nutrition, personal hygiene, or managing finances, which puts the individual's health and safety at risk. 3)At risk intervention by judicial system, mental health or social services because of inappropriate behavior. 4)Difficulties in cognitive abilities such as recognizing personal danger or inability to recognize inappropriate social behavior.

Description

At Strategic, our clinical staff is prepared to address the difficulties which challenge those with mental health problems. Designed to prevent institutionalization and repetitive inpatient stays, mental health support services work closely with the client, forming a service plan which address unique needs and personal goals. Problem areas are targeted and addresses. New skills are taught, enabling a higher functioning lifestyle. The client's counselor will work with him in his home as well as in the community. We strive to make an independent, healthy lifestyle, which we believe every person should have the right to pursue, an attainable goal.
