Horizon’s ICF programs offer adults with a primary diagnosis of intellectual disability with active treatment in functional skills to create an environment in which the individual can not only live safely but can excel. Horizon’s ICFs also support individuals with a variety of medical and behavioral needs that require extensive supports to be healthy. High quality, comprehensive services are provided by Horizon's interdisciplinary team of ICF staff which includes doctors, nurses, psychologists, dieticians, recreational therapists, OT/PT speech therapists, residential technicians, counselors, family social workers, case managers, and citizen volunteers among others. All work together to monitor Service Treatment Plans, medical needs, and provide daily living support which includes full-service laundry, cooking, cleaning, grocery shopping and more. The result is the ability to provide continuous, direct care twenty-four (24) hours per day, with 24/7 oversight and on-call availability after normal business hours.

The 128-bed facility is organized into communities (64 beds), with two neighborhoods (32 beds). Each neighborhood includes two households (16 beds) with adjoined access to a courtyard. The households feature all private rooms each with its own bathroom and walk in shower as well as a spacious dining room, kitchenette, and dayroom. This person-centered approach creates a true sense of community and home, which supports providing our Veteran residents the opportunity to have as good or better quality of life than before they came to us.

To find out more information on how to start a group at your child's school, please call Good Sam. Living With Loss - Support group for adults grieving the loss of a loved one. Weekly sessions run for 8 weeks and combine education and group activities for discussion. Class is held four times per year. Pre-registration is required. Hope & Hot Chocolate - Held throughout the holiday season to help bereaved persons cope effectively with the demands of the holidays. Women and Men's Lunch Groups - An opportunity for those who have experienced the loss of a spouse or partner to get together for casual lunch and conversation. Call for meeting places/dates/times for Roanoke and NRV locations. Puzzle Pieces - Drop-in support group for bereaved persons interested in understanding grief better and being able to talk with others dealing with loss. The group is facilitated by hospice staff who help group members identify where they are in their own grief journey and gain new insights or coping tools. Knitty Gritty Grief Group - This grief support group combines knitting and crocheting and grief support. It allows each member to share their loss in a safe and compassionate environment while providing support through therapeutic interaction. After completing a project (e.g. prayer shawl, lap blanket, etc.), each member may keep their knitting or donate it to a hospice patient.
