Case Management provides assistance to individuals with severe mental disabilities to obtain the services they need to live productively in the community. Because services can be fragmented in many areas, case managers provide a critical function to monitor a person's needs and assure that appropriate agencies get involved.

The Case Manager / Support Coordinator will assist the individual and family by linking or connecting them with the identified, available service provider(s) or resources.

Community Support Programs-Case Management is composed of programs designed to provide community based Behavioral Health services to seriously mentally ill adults and those with substance use disorders Program activities support consumer recovery and community integration by providing a continuum of services designed to promote maximum independence in the community. Services are tailored to respond to the unique needs of individuals with serious mental illness.

Outpatient mental health and substance abuse treatment programs for children, adolescents and adults are provided at Scott County Mental Health Center. Services include individual and group psychotherapy, marital and family counseling, psychiatric services, medication monitoring, crisis services, intensive in-home services, and education and consultation services. Project LINK, an outpatient program for pregnant, substance-abusing women or mothers with dependent children, is also offered at this site. In-Home Services, Home-based family-focused services are provided for children from birth to 17 with emotional difficulties, returning from placement, or who are at risk of out-of-home placement or other services. FOOTHOLDS works one on one with children and their families to build a healthy family environment. The program helps identify obstacles, increase communication skills, structure positive home, social skills development; provides support, guidance and links to resources; and educates caregivers. Promotes and enhances independent living of adults aged 60 and older by stabilizing their mental illness or emotional disturbance. All individuals are involved in day programs and actively participate in community events. Each individual assists with the development of a service plan based on his/her individual preferences.
