Case Management / Support Coordination programs coordinate the delivery of services for persons with behavioral health, substance use and intellectual/developmental disabilities. This includes a comprehensive assessment of individual needs in the areas of housing, vocational training/ employment, physical health, psychological / psychiatric treatment, and other life domains. This assessment includes the compilation of developmental, educational, vocational, social, and psychological evaluations as well as information from the person's family and other service providers.
Once the individual has been assigned a Case Manager / Support Coordinator, a service plan is prepared which includes the strategies most appropriate for meeting each individual's needs. The Case Manager / Support Coordinator will assist the individual and family by linking or connecting them with the identified, available service provider(s) or resources.
