CrossOver specializes in the treatment of chronic diseases, proving comprehensive treatment plans for patients suffering from diabetes, hypertension, chronic obstructive pulmonary disease, cardiovascular disease, and other chronic conditions through the Chronic Disease Management Program. Services are eligible to individuals in the town or county of Culpeper, who have a family income less than 250% of the federal poverty level, and who are without health insurance. Services are provided by qualified health care professionals and patients are asked to make small donation to help cover the cost of prescription medicines.
