The Medical Care for Children Partnership (MCCP) is a community partnership dedicated to providing medical and dental services to children of Fairfax County who otherwise are ineligible to receive healthcare offered through Medicaid or other private and public sources.

Provides a medical clinic for residents of Gloucester, Lower King & Queen and Mathews who do not have health insurance or who have certain forms of medicaid. Services include a primary and chronic care clinic, dental clinic and prescription services.

Medical clinic services are for adults residents who are below 300% of the federal poverty level and do not have health insurance.

Provides free medical and dental services and free or low-cost prescriptions to uninsured residents of Smyth, Washington and Grayson counties who are at or below 300% of the Federal Poverty Level.

A free primary care medical clinic serving low-income uninsured residents of Carroll, Grayson, and City of Galax. Will assist those with Medicare Part A & Medicaid.

The Free Clinic provides a full array of primary and specialty medical services for adults, either at the Free Clinic or through referral to our partner community providers. The Free Clinic also provides laboratory, diet and nutrition counseling, health education classes, support groups and behavioral health services.

Free Clinic serving Patrick County. Services include: - Two Medical Doctors, one Family Nurse Practitioner, one Physician's Assistant and one Women's Health Nurse Practitioner provide primary care. - A Licensed Practical Nurse serve to educate the patient, assist the medical staff and perform laboratory tests. - Clinical Practitioners of Osteopathic Medicine from Virginia Tech are available each month for patients. - Dental Hygienists serve to triage critical dental issues when ordered by our physicians. - MedAssist is located at the clinic to assist patients with the costs of maintenance medications. - Staff may assist patients with applications for financial assistance for facilities outside the clinic. - Appointments with local dentists and optometrists are available when precribed by the medical staff.

Bassett Family Practice is a federally granted community health center that provides comprehensive medical treatment to people of all ages and is equipped to provide many of the services ordinarily found in a private care physician's office.

Pathways provides resources through an on-site medical clinic, visiting dental care, wellness workshops, advocacy and referrals, and counseling and support for mental and physical challenges and addictions.

Community clinic providing primary and preventive health care services including physical examinations with early screening for health and developmental problems, chronic disease management especially diabetes and hypertension , immunizations, prenatal and gynecological care, family planning, HIV/AIDS screening and counseling, education and family support, information about and referral to community resources and medical specialists, case management, pediatrics, also adult and pediatric dentistry.

The Ambulatory Care Clinic (ACC) is located off-site from Sentara Norfolk General Hospital. The ACC offers lab services, X-rays, and EKG's are also available for diagnostic work-up. A Medication Assistance Program is available to help patients get prescription medications free or at a reduced cost. External referrals for specialty care are also made to other Sentara providers in the community.
