Must have lived in service area for the past 6 months, be uninsured, not eligible for Medicare, Medicaid or Veterans benefits, with household income not exceeding 125% of federal poverty guidelines.

9:30am to 12pm on Mondays, 11am to 2pm on Tuesdays, and 9:30am to 4:30pm on Wednesdays and Thursdays

Application process
Applicants must bring proof of household income, have been a resident for a minimum of six months (Carroll, Grayson, or City of Galax) and be totally uninsured. Will service individuals with Medicare Part A & Medicaid.
Fees
Yearly fee of $35.00 for first year, $25.00 each subsequent year.
$25.00 per visit for dental appointments
