Case Management - Harrisonburg, Brain Injury Connections of the Shenandoah Valley

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.

Didlake Day Support Services provide opportunities for people with disabilities to optimize their personal, social, and vocational skills. Our services focus on personal growth and learning to help each individual live a full, responsible, productive life. Programs Community Engagement The Community Engagement program provides opportunities for participants to directly interact with people who do not have disabilities in naturally occurring community settings. While in the community, program participants are supported and encouraged to learn and understand how to successfully engage in each activity with the goal of developing interdependent relationships. Group Day The Group Day Program provides on- and off-site activities that teach individuals to be independent, healthy, and active participants at home and in the community. Didlake work with each participant to develop and meet their specific needs and goals. Rehabilitation Teaching & Independent Living (RT/IL) Services help people who are blind, vision impaired, or deafblind learn skills to live more independently.
