Mental Health Skill-building Services (formerly titled Mental Health Support Services) are provided for individuals 18 years and older who suffer from serious mental illness or mental illness with intellectual disabilities or substance abuse. This home and community-based program includes training, assistance and monitoring for individuals with a mental health diagnosis and who meet eligibility criteria.

Services are designed to meet the needs of clients experiencing difficulties in their lives due to emotional, behavioral, and mental health issues. Provide transitional support and stabilization services to clients returning to the community from a more restrictive environment. The goal is to provide each client with individualized services and training so they can function as independently as possible. Each client will learn to utilize appropriate community resources to maintain stability in the least restrictive community setting.

Provides a Retirement Community for Independent Living and Assisted living for seniors age 55+.

The purpose of LIFT is to provide apartment-based Independent Living and Support Services to help foster care youth, ages 17-21, transition from residential care to independent living. Youth receive housing, vocational and educational services, life skills training, therapeutic services (counseling, therapy and/or substance abuse education), and the development of a network of support services in the community. LIFT is licensed for 25-beds for males & females transitioning from the foster care system to independent living in the community.

The center provides a comprehensive range of services tailored to the specific needs of the community, including food, clothing, addiction recovery support, computer training, educational programs, health clinics, job training/placement, legal aid, ESL classes, life skills, and parenting classes.

Lynchburg Area Center for Independent Living promote a philosophy of consumer (ie people with disabilities) control, peer support, self-help, self-determination, equal access, and individual and system advocacy, to maximize the leadership, empowerment, independence, productivity of individuals with significant disabilities into the mainstream of American society.

Foster teens who are at least 16 years of age. This program varies from year to year depending on the needs of the teens and the funding available from the State. Based on needs identified through an Independent Living Needs Assessment, the teens receive counseling and participate in group activities regarding subjects that will assist them in becoming competent adults and self-sufficient, productive citizens.

The program provides group homes for individuals with mental illnesses, intellectual disabilities, substance use, and co-occurring disorders. Also provides a highly structured program for residents to make progress toward more independent living. It operates 24 hours a day and seven days a week. The program is internally accessible for the handicapped.

Meadowcreek Day Activity provides training and assistance to adults with developmental disabilities to acquire, improve and maintain functional abilities, while also achieving social integration with peers and the community. Meadowcreek Day Support provides opportunities for individuals to pursue personal interests and strengths in the community through art, music, technology, self-advocacy, and wellness. Meadowcreek coordinates with a variety of community-based services, including Charlottesville Parks & Recreation, Access Arts, Madison House, Meals On Wheels, Best Buddies, and Salvation Army.

Affordable, non-medical independent living housing offering private and semi-private rooms in shared homes. Provide safe, stable living environment for adults who can living independently but benefit from structure and community support.

Works to empower people to take charge of their own lifestyles. Designed and operated by people with disabilities. Promotes leadership, empowerment, independence, and productivity of people with disabilities. Works with individuals and with the community, to remove barriers to independence. This is a satellite office. Services include I & R, Advocacy, Peer Counseling, Independent Living Skills Training, Nursing Home and Youth Transition Services. Equipment for hearing impaired such as telephones, doorbells, pocket talkers, etc. Asistance for obtaining eyeglasses.

DBVI provides services and resources which empower individuals who are blind, vision impaired or deafblind to achieve their desired levels of employment, education, and personal independence. Services are provided through 4 main programs: Vocational Rehabilitation, Rehabilitation Teaching and Independent Living, Low Vision Services and Education Services. Most services are provided in the person's home, school, or workplace. Referrals are accepted from individuals, families, schools, doctors, and other community partners.

Retirement village include 14 individual 2-bedroom privately-owned cottages for independent living and a 60 bed assisted living facility with 20 additional beds for residents with Alzheimer's or a related dementia.

Rehabilitation Teaching & Independent Living (RT/IL) Services help people who are blind, vision impaired, or deafblind learn skills to live more independently. Trained staff visit individuals in their homes to talk about services and support that can help with reading and writing, using a computer, and taking care of everyday tasks. They also teach skills for personal care, socializing, getting around safely, and enjoying hobbies and other activities. The goal is to help each person gain confidence and independence in their daily life.

A residential program serving people with intellectual disabilities and other secondary disabilities such as emotional/behavioral problems or physical disabilities. Intensive long-term community residential program. Homes are available in Carroll and Wythe Counties and the City of Galax, VA.

Offers instruction in independent living skills including personal finance, cooking and housekeeping, instruction in Braille keyboarding, orientation and mobility training, the use of white canes, and transportation systems. Services provided include teaching techniques that enable individuals with visual impairments to maintain and retain independence.

Didlake Day Support Programs provide opportunities for people with disabilities to optimize their personal, social, and vocational skills. Programs and services focus on personal growth and learning to help each individual live a full, responsible, productive life. Provides a supportive environment and encourage individuals to have new experiences. All individuals are active participants along with their family members, support coordinators, and direct caregivers in the development of their specific program plan. Program plans support the individual needs, desires, and interests of each participant.

The individuals in the Community Support Program receive drop-in services from our dedicated and highly trained Direct Support Professionals. These individuals live in their own homes or in their family’s homes. Our DSPs provide personalized services that enable them to navigate their community and manage their everyday life.

Provides an independant senior living apartment complex for older adults. Apartments are one bedroom only.
