Death and birth certificates, as well as marriage and divorce records, can be obtained from the local county health department. Vital Records are available to immediate family members only- mother, father, husband, wife, child, brother, sister and grandparents with valid ID. Birth records can be issued to the legal guardian with proof of custody papers. Aunts, uncles, cousins, in-laws, etc. cannot obtain a Vital Record.

Provides community based services to people who have intellectual disability. Agency has 5 licensed group homes: Pickering Place, The Apartment, Corson House, Clifford and Liberty. Oxford House is a network of self-run, self-supported recovery homes for members recovering from alcohol and substance abuse. When an individual is accepted for membership in an Oxford House, there is no time limit on how long he or she can live there, but the use of alcohol or drugs or non-payment of rent will result in expulsion. There is no clean time required to move into an Oxford House.

Provides eligible families with a monthly cash payment to meet their basic needs. For a child to be eligible he must be: - Under age 18, or if 18, will graduate from high school before age 19; - Going to school regularly if he is between the ages of five and 18; - Living with a parent or other relative; - A United States citizen or an eligible immigrant. Emphasizes personal responsibility. Participants may be provided with services such as job skills training, work experience, job readiness training, child care assistance, transportation and other work related expenses. A child will not be eligible if born to or adopted by a TANF recipient more than 10 months after an applicant begins to receive TANF payments. The Division of Child Support Enforcement will send all support collected for this child directly to the family. This support will not count as income in the TANF program.

Horizon’s ICF programs offer adults with a primary diagnosis of intellectual disability with active treatment in functional skills to create an environment in which the individual can not only live safely but can excel. Horizon’s ICFs also support individuals with a variety of medical and behavioral needs that require extensive supports to be healthy. High quality, comprehensive services are provided by Horizon's interdisciplinary team of ICF staff which includes doctors, nurses, psychologists, dieticians, recreational therapists, OT/PT speech therapists, residential technicians, counselors, family social workers, case managers, and citizen volunteers among others. All work together to monitor Service Treatment Plans, medical needs, and provide daily living support which includes full-service laundry, cooking, cleaning, grocery shopping and more. The result is the ability to provide continuous, direct care twenty-four (24) hours per day, with 24/7 oversight and on-call availability after normal business hours.

Provides mobility assistance for elderly and disabled. Provides financial assistance to purchase vans, scooters and new and used vehicles that are handicapped accessible. Rental vans available. Offers a wide range of products, including Lifts, Scooters and Portable Ramps and has a fully trained service staff specializing in van conversions.

On recommendation of the Lancaster and Northumberland Departments of Social Services and clergy, provide nutritious, non-perishable food to last each family member about a weeks.
