Adult Day Care - Haysi, Generations at Haysi, Appalachian Agency for Senior Citizens

Bridges the gap between children and older adults in a safe, homelike setting. Provides preschool and before and after school programs, all in an intergenerational atmosphere. Together, children and adults have participated in special activities, including a pen pal program and a celebration of our Appalachian heritage. Integrates the wisdom and insight of the elderly with the energy and enthusiasm of the young. Intergenerational day care offers children the opportunity to take part in carefully selected, supervised activities with senior citizens. It also allows family members to work or take a break from caring for a loved one. Skilled staff create individual care plans for participants. Staff includes a supervisor, activities director, registered nurse, and personal care attendants. Participants receive recreation, socialization, health monitoring and medication management in a home like environment. Each day we serve a USDA-approved breakfast, lunch, and afternoon snack. Collaborate with the AllCARE for Seniors PACE program. Through this partnership, participants receive socialization, in conjunction with therapy and medical services while they attend day care. Generations continues to offer training and support sessions to caregivers throughout our service area.

Adult Protective Services - Galax, Galax City Social Services

Investigates reports of abuse, neglect, and exploitation of adults sixty years of age or older and incapacitated adults age eighteen or older. If protective services are needed and accepted by the individual, local Adult Protective Services social workers may arrange for a wide variety of health, housing, social and legal services to stop the mistreatment or prevent further mistreatment. Services offered may include home-based care, transportation, adult day services, adult foster care, nutrition services and legal intervention in order to protect the adult. Services may also be arranged for individuals in emergency situations who lack the capacity to consent to services.

Long Term Care Pre-Admission Screening - Roanoke, Roanoke City Health Department

This program provides a timely screening assessment for Medicaid or Medicaid eligible clients needing nursing home placement or services in the home or community. The target population for this program includes the elderly, disabled adults, or children needing a waiver for long-term placement or services for home-based care. Screenings are conducted in partnership with the Department of Social Services (DSS) and requests for screenings need to be made through DSS. A nurse and a social worker screen the client to determine if they meet Medicaid's criteria for either (1) Functional capacity: the degree of assistance an individual needs to complete the activities of daily living (bathing, toileting, dressing) or (2) Medical or Nursing need (wound care). Services may include the following: in-home care, adult day care, or admission to a care facility. Referrals for screenings may be made through your local Health Department.

ADRCs, also known as No Wrong Door agencies in Virginia, provide a source for the public to receive accurate, unbiased information on all aspects of life related to aging or living with a disability. Using a "No Wrong Door" concept, ADRCs address many of the frustrations individuals and their families experience when trying to find needed information, services, and supports. Through integration or coordination of existing aging and disability service systems, ADRC programs raise visibility about the full range of options that are available, provide objective information, advice, counseling and assistance, assist people in making informed decisions about their long term supports, and help people more easily access public and private long term supports and services programs, performing in a person-centered way.
