Case Management-Mental Health - Roanoke, Mental Health Services, CHIP of Roanoke Valley

An early intervention program, CHIP addresses the unique mental health risks children and families in poverty face, through in-home family support and parenting education that helps lessen the detrimental effects of poverty. Families receive support and encouragement, therapeutic interventions, parenting guidance, referrals for counseling or out patient counseling through CHIP either in-home or in an office setting, care coordination for psychiatric evaluation and medication management. When combined, these interventions produce confident parents, self-sufficient families, and healthy children with a strong foundation upon which to build future successes. Mental health services focus on mothers with mental health issues (diagnosed or presenting) and children through age seven who are at risk for serious emotional disturbance, as well as children who are already showing signs of emotional or behavioral issues.

Mental Health-Outpatient - Weber City, Scott County Behavioral Health Services, Frontier Health

Outpatient mental health and substance abuse treatment programs for children, adolescents and adults are provided at Scott County Mental Health Center. Services include individual and group psychotherapy, marital and family counseling, psychiatric services, medication monitoring, crisis services, intensive in-home services, and education and consultation services. Project LINK, an outpatient program for pregnant, substance-abusing women or mothers with dependent children, is also offered at this site. In-Home Services, Home-based family-focused services are provided for children from birth to 17 with emotional difficulties, returning from placement, or who are at risk of out-of-home placement or other services. FOOTHOLDS works one on one with children and their families to build a healthy family environment. The program helps identify obstacles, increase communication skills, structure positive home, social skills development; provides support, guidance and links to resources; and educates caregivers. Service in Scott County is provided in partnership with Planning District One Behavioral Health Services.

Case Management / Support Coordination programs coordinate the delivery of services for persons with behavioral health, substance use and intellectual/developmental disabilities. This includes a comprehensive assessment of individual needs in the areas of housing, vocational training/ employment, physical health, psychological / psychiatric treatment, and other life domains. This assessment includes the compilation of developmental, educational, vocational, social, and psychological evaluations as well as information from the person's family and other service providers. Once the individual has been assigned a Case Manager / Support Coordinator, a service plan is prepared which includes the strategies most appropriate for meeting each individual's needs. The Case Manager / Support Coordinator will assist the individual and family by linking or connecting them with the identified, available service provider(s) or resources.
