BreakThru Services provides intensive in-home services which offers goal-directed training to enable individuals with mental, behavioral and or emotional illness to achieve and maintain stability and in-home placement in the most appropriate, least restrictive environment. BreakThru Services LLC will also provide training in the following areas to the individuals we serve: Training in: Benefits of psychoeducation in the home setting Modeling of appropriate behavior related to the individual's health and safety Functional skills and appropriate behavior related to the individual's health and safety Therapeutic Interpersonal relations between family members in the homeActivities of daily living

Home based services are designed to provide convenient, specialized support for families in need. Offers in-home therapy, case management, and crisis support for those who cannot access traditional mental health services

Outpatient mental health and substance abuse treatment programs for children, adolescents and adults are provided at Scott County Mental Health Center. Services include individual and group psychotherapy, marital and family counseling, psychiatric services, medication monitoring, crisis services, intensive in-home services, and education and consultation services. Project LINK, an outpatient program for pregnant, substance-abusing women or mothers with dependent children, is also offered at this site. In-Home Services, Home-based family-focused services are provided for children from birth to 17 with emotional difficulties, returning from placement, or who are at risk of out-of-home placement or other services. FOOTHOLDS works one on one with children and their families to build a healthy family environment. The program helps identify obstacles, increase communication skills, structure positive home, social skills development; provides support, guidance and links to resources; and educates caregivers. Service in Scott County is provided in partnership with Planning District One Behavioral Health Services.

VFS Intensive In-Home Services are designed to improve family dynamics, preventing out of home placement and assisting those returning home from out of home placement (group home, foster home, juvenile detention, emergency shelter, psychiatric hospitalization and residential facilities) other components of service include providing therapeutic interventions, family and individual therapy, care coordination and emergency assistance.

Provide intensive in-home counseling services for adolescents that are at risk of removal from the home. Provide case management services to adults that need assistance with ADL's. Provide counseling services for adolescents who have sexually offended.

Comprehensive programs including assessments and referrals to assist in everyday functioning and recovery. Counseling services can be individual, group, and/or family psychotherapy. Offer two types of outpatient services dependent upon client need: regular outpatient and intensive outpatient. Can also help provide links for medical care to adults diagnosed with serious mental illnesses. Also offer Group Therapy and Children's Support Services.

Provides behavioral health treatment which include mental health counseling, substance abuse services, ABA and foster care. Offer in-home and outpatient services for at risk youth, families, and communities, with the steadfast goal of promoting reunification and/or permanency goals for all parties involved.

The Behavioral Health Services Program provides counseling services to children, adolescents, adults, families and organizations. Intensive In-Home Services provide support, guidance, and skills training for children who are at risk of being removed from their home due to serious behavioral issues. The Villa works with families to find fresh approaches to breaking the cycle of destructive behaviors. Services provided include: • Crisis intervention and diagnostic assessments • Collaboration with parents/guardians to create an effective, positive behavioral management system • Caregiver training and support • Individual and family counseling • Care coordination with schools and community resources to create a stable environment • Individualized approaches that focus on the unique needs of the child and family • A 24-hour emergency response system, including weekends • Comprehensive discharge planning • Referrals for additional services as needed

Provides in-home behavioral and mental health training and support to individuals with impairments in major life activities and mental, behavioral, or emotional disorders.

Connections ABA & Family Support Services Provides home-based and clinic-based Applied Behavior Analysis (ABA) services for children and adolescents under the age of 21 who demonstrate significant impairment in adaptive functioning that is related to either developmental delays or other health conditions.

CHCG provide individualized training and support to individuals in-home or in a comfortable non-restrictive environment. Assist in developing real world problem solving techniques to encourage independence, personal and community stability. Services will be provided in three month increments with a maximum of ten hours weekly. The Child and Family Services center provides prevention, substance use and mental health services to the children and families in the community. The goals of these services are to help families build resiliency and keep children/youth safely in their homes and communities when possible. Trauma-informed services and prevention trainings are provided in the communities, schools and in an office-based environment.
