Residential Services provides housing opportunities for individuals needing specialized services and supports to live in the community. This program provides personal, social and behavioral skills training to improve an individual’s quality of daily life. The program’s primary focus is to facilitate independent living and self-empowerment, while operating in accordance with Department of Behavioral Health and Developmental Services (DBHDS) Human Rights and Home and Community-Based Services (HCBS) Rights regulations. Each individual has a private bedroom.

Polly Summit is a residential facility for 10 adults with serious and persistent mental illness that helps them achieve independence within the community. Clients are offered support for community living, psychosocial support services, and are taught daily living skills. Service in Wise County is provided in partnership with Planning District One Behavioral Health Services.

Provides 24 hour a day supervision, training, and assistance in daily living skills for adults who have a developmental disability. All individuals are involved in day programs and actively participate in community events. Each individual assists with the development of a service plan based on his/her individual preferences.

Horizon’s ICF programs offer adults with a primary diagnosis of intellectual disability with active treatment in functional skills to create an environment in which the individual can not only live safely but can excel. Horizon’s ICFs also support individuals with a variety of medical and behavioral needs that require extensive supports to be healthy. High quality, comprehensive services are provided by Horizon's interdisciplinary team of ICF staff which includes doctors, nurses, psychologists, dieticians, recreational therapists, OT/PT speech therapists, residential technicians, counselors, family social workers, case managers, and citizen volunteers among others. All work together to monitor Service Treatment Plans, medical needs, and provide daily living support which includes full-service laundry, cooking, cleaning, grocery shopping and more. The result is the ability to provide continuous, direct care twenty-four (24) hours per day, with 24/7 oversight and on-call availability after normal business hours.

Provides residential services for adults and children with special needs and disabilities including intellectual disabilities, autism, pervasive developmental disorders, mental illness and emotional disturbances. Services include training in daily living, community, domestic, leisure, behavioral, and social skills with access to therapeutic supports including individual and family therapy, speech/language therapy and occupational therapy. Adult and Youth Group Homes are located throughout Frederick and Clarke counties as well as in Winchester and Richmond Virginia Does not accept individuals who are active fire-setters, actively suicidal, active substance abusers, homicidal, or sex offenders. Students and adult consumers are identified as having more than one diagnosis; for example, autism with severe mental retardation; anxiety disorder and mental retardation; and bi-polar disorder and learning disabilities. Grafton also serves people with physically handicapping conditions such as cerebral palsy.

Pathway Homes provides individuals with mental illness and co-occurring disabilities a variety of non-time-limited housing and services to enable them to realize their individual potential.

Also provides a highly structured program for residents to make progress toward more independent living. It operates 24 hours a day and seven days a week. The program is internally accessible for the handicapped.

Chaplin provides 24 hour supervision in a safe and structured environment, in which residents are empowered with behavior modification tools that facilitate the development of healthy and productive patterns of behavior.

The Elk Hill-Amani Group Home serves a maximum of eight court and social services referred adolescents' males ages 14 - 17 who are no longer at high risk for violence, severe aggression, sexual offense and elopement, as well as serving as a step down from the residential program. Set in a supportive community environment, daily routines are structured around independent living and transitional issues.

Offers structured therapeutic groups with home independent living programs for male and female adolescents as well as aftercare guidance services for youth transitioning into the community. They provide information and referrals statewide for people they cannot serve.
