Weekday enrichment and engagement for older adults living with disabilities and/or dementia in Charlottesville and Louisa. Individuals outside these areas can also become members. The JREC program provides activities, healthy lunch, intergenerational connections, caregiver support, and health/medication management.

A 98-bed assisted living facility for adults who require assistance with at least two activities of daily living has physical or mental impairment, not capable of self-preservation without the assistance of others, and respite care. Will accept couples and Alzheimer's patients.

Camp Bruce McCoy is an annual 2 week residential camp for adults with brain injuries held in May-June. The camp is held in Chesapeake. Call for details.

Serves adult men & women with the primary diagnosis of Intellectual Disability (diagnosis must be documented as originating prior to age 18). Provide care, assistance and supervision to individuals in their own homes. These services are supplemental to the care provided by a primary caregiver or they may support an individual whose level of independence does not require a primary care provider. Each resident has a plan of care outlining support needs. This plan outlines specific outcomes expressed by the individual or evaluated as an area of need/challenge. The plan is used to help each individual progressively strengthen his/her own individual knowledge, skills and independence.

Encourages and facilitates participation in individualized activities, ensures safety, and provides needed practical support such as the administration of medication, transportation to community outings, etc. An overall focus on maximizing potential and skill-building is at the core of day support values. Some specific skills that is supported include self-care and hygiene, eating, toileting, task learning, community resource utilization, behavioral, social, as well as opportunities for peer interaction and community integration. OPTIONS centers offer: Life skills room to learn how to make a bed, iron, hang clothing, vacuum, and operate everyday appliances Flower and vegetable gardens Game room Book club Outdoor spaces for walking, relaxing, bird watching, and playing basketball Community Engagement activities (when possible): visiting local libraries, volunteering at local churches and Meals on Wheels, touring local museums, going to the movies, having lunch at local restaurants

Long term care nursing services that provides physical, speech and occupational therapy. Offers respite care.

Provides non-medical services such as companionship, meal preparation, medication reminders, light housekeeping, laundry, errands, incidental transportation and personal care, which includes, bathing, feeding, incontinence assistance and transferring.

Our Vacation & Recovery Stay Program is ideal for: * Family Members who need to take a break from their everyday responsibilities as caregivers. Our program affords family members the opportunity to vacation, travel, deal with business or personal affairs, or simply take a needed rest from the stress and strain of round-the-clock caregiving. * Patients who have completed a hospital stay or rehabilitation, but feel they would benefit from an added period of professional care before returning home. * Those who have undergone same day surgery or other outpatient procedures and want a day or two of rest before resuming their daily routines. * For assisted living, this program also gives prospective residents and their families the opportunity to evaluate the benefits of assisted living at Richfield before making a permanent move.

Provides assisted living services (respite care) on a short term basis to seniors needing care after surgery, a fall, an illness, during inclement weather periods or when family members are away or just need a short break or to experience assisted living on a trial basis.

Offers a structured program, including recreational and leisure activities, crafts, music, field trips, gardening, outings, guest speakers, life skills, coaching, computer labs and exercise classes. Nutritious, diet specific lunches along with two healthy snacks are provided daily. The Registerd Nurse(RN) provides monthly health assessments and ongoing medication management as ordered by physicians. Assistance, as may be ordered, with activities of daily living are provided; such as: feeding, toileting, transferring and ambulation. Bathing and beauty/barber services available. Daily and monthly progress notes are kept by Certified Nursing Assistants and Nursing Assistants (CNAs/NAs) and the Program Coordinator.

BrightSteps Homecare LLC provide in-home care services that promote independence, dignity, and comfort for seniors and individuals with health challenges. Services include personal care, companion and respite support, skilled nursing oversight, and post-hospital assistance.

Patients may be admitted to the hospice house for acute management of symptoms, for the final days of life or for respite care. Short-term services are geared towards those who would like to stay in our facility for a temporary period of time. Sometimes, your loved one needs a change of routine, or his or her caretaker needs a break. Whatever the case may be, we welcome residents for any period of time (minimum of four days), whether it be a week to several months. Short-term facility offers many of the same services as our long-term care, including: 24-hour nursing care Recreational Services Dining Services Transportation Access to Amenities

This respite program can provide up to 720 hours per year for qualified patients in the Medicaid program. Camp Baker offers a day summer camp, weekend and emergency respite, for children and adults with developmental disabilities. The camp is located on a 22-acre wooded site in Chesterfield County, VA. Year-round and seasonal recreation and training activities include swimming, music, hiking, arts and crafts, community outings, horseback riding, and canoeing.
