Companion care, chore and homemaker services are offered for the very low-income client. Companion services consist of paying an individual an hourly rate to assist the client. The companion may assist with shopping, meal preparation, light housekeeping, provided supervision if needed, and attend to some personal care needs (such as assisting with bathing). Chore services are non-routine, heavy home maintenance tasks that may include window washing, floor maintenance, painting , chopping wood, snow removal, and minor repair work in the home. Homemaker services include instruction in or provision of activities to aminatin a household and may include personal care, home management, household maintenance, nutrition, and consumer and health care eduation. Adult services also include assisting the adult and family members in making long-term care living arrangements for the adult. This could include referral for an adult home, nursing home, or the decision to reside with a family member. The social workers assist the families in making the best choice for their own individual circumstances.

Offers companion services for limited hours each week, to assist SSI or low income clients over 18 with personal care or light housekeeping tasks. Seeks to prevent institutionalization.
Services include but are not limited to: Companion/sitters/safety supervision, light meal preparation, light housekeeping, transportation to physician and dental appointments, social events, etc. medication reminders, and care coordination. Accept clients on a 24/7 basis or a minimum of 3 hours per day.

The Home-based Care Program provides in-home assistance for low-income adults over the age of 60 and/or adults age 18 and above who have severe disabilities that prevent them from performing these tasks themselves.
Adult Services are provided to disabled persons who are eighteen years of age and older and to persons sixty years or older. Adult Services includes the provision of case management, home-based companion services, screening for Medicaid community-based care and nursing home placement, screening for admission to Assisted Living Facilities, and other activities to aid the adult and their family.

Will dedicate the time needed to help with lifes chores or play a game over a cup of tea. *Home Health Aides are the right choice for people who have a hard time managing the more basic activities of daily living. Assists with everything from personal hygiene to taking blood pressure and help feeding.

Persons age 60 and older may benefit from a trained and supervised aide to help with light housekeeping tasks, grocery shopping and errand running. This service may average 3 hours per week and offer respite to a caregiver.
Home and community-based services allow the elderly or adults with disabilities to remain in their homes rather than move to institutional placements. Screening for adult care residence and nursing facility placements is provided.
Services provided by Senior Companions include daily tasks such as companionship-talking and listening, transportation, peer- counselling, grocery shopping, meal preparation and light housekeeping.

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to : assisting in placement such as in an adult home, nursing homes, etc.; transportation when needed; companion services designed to assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization.
