The goal of the SSVF Program is to promote housing stability among very low-income Veteran families who reside in or are transitioning to permanent housing. SSVF provides outreach and case management services, and will assist participants to obtain VA benefits and other public benefits. SSVF may be able to provide temporary rental and/or security deposit assistance to veterans who are at imminent risk of losing their housing while also meeting the required vulnerability targeting criteria. In order to be considered for assistance, veterans must call the SSVF Intake Coordinator at the main office line and complete the screening questionnaire. SSVF is unable to assist veterans who have a dishonorable discharge.

Provides services necessary to assist the patient and family during hospitalization and after discharge. The department attempts to assist the patient in making the best possible use of medical and social resources available.

Support services for adults with developmental disabilities and their families. The JFS Special Needs Case Manager provides assessment, evaluation, and monitoring of your family member with special needs to ensure maximum safety, inclusion, and independence. If needed, adult guardianship services are available through the JFS Personal Affairs Management program.

Uses Coleman Transition Intervention to help people after they are discharged from a health care facility. Following hospital discharge, patients often require care from different medical professionals in many different settings. This program is designed to improve the continuity of care for persons with chronic conditions across different health settings and ensure that each transfer between setting goes smoothly. This is a FREE service to assist people to recover, understand and manage your medications better, prevent hospital re-admissions, and plan for a follow-up appointment with your primary care physician.

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to : assisting in placement such as in an adult home, nursing homes, etc.; transportation when needed; companion services designed to assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization.

The Care Coordination Program assists older adults (and their families) by helping to coordinate and organize the care they are receiving from various agencies and organizations. Coordinators provide information and referrals; collaborate with other service professionals; and develop person-centered service plans that focus on participants' needs, strengths, and preferences. They also follow up with individuals each month to ensure those plans are working well and adjustment them if any needs have changed.

This program offers temporary assistance to at-risk, low-income veterans and their families in an effort to keep them from becoming homeless. This grant program is designed to house these at-risk families as quickly as possible by providing legal counseling and financial aid for rent, utilities, moving costs, childcare and bus transportation, employment assistance and benefits counseling.

Our dedicated Care Coordinators work closely with older adults to understand their strengths, needs, and preferences. They provide assistance with referrals and follow-up as necessary to ensure a tailored and effective care plan.

Professionally certified care managers assess older person's living situations and provide cost effective options to meet their needs. Client fees based on ability to pay. Develops a plan of care, in home assistance arrangements, meals on wheels, assistance with complex paperwork from other agencies, Alzheimer's and Dementia support groups, adult day care, home repair and weatherization, health insurance counseling, legal aid, friendly visitor, telephone reassurance, and transportation.
