A Medicaid approved program provided by the Alexandria Health Department to foster safe and healthy pregnancies and healthy babies by coordinating services and providing education.

Adult Services are provided to adults age 18 and older who have a disability and to their families when appropriate. Adult Services may include the provision of case management, home - based care, transportation, adult day care, nutrition services, placement services, long term care pre-screening for Medicaid-funded services, guardianship monitoring, and other activities to aid the adult.

The Care Coordination Program assists older adults (and their families) by helping to coordinate and organize the care they are receiving from various agencies and organizations. Coordinators provide information and referrals; collaborate with other service professionals; and develop person-centered service plans that focus on participants' needs, strengths, and preferences. They also follow up with individuals each month to ensure those plans are working well and adjustment them if any needs have changed.

Assesses client needs and develop coordinated plans of care that use community and family resources. Nutritious home delivered meals along with friendly phone calls by volunteers are among the services that can help many elderly people maintain independence.

Adult Services are provided to impaired adults 18 years of age and over adults 60 years of age and over. The scope of services is intended to maximize self-sufficiency, to prevent abuse, neglect, and exploitation, to prevent, delay and/or reduce inappropriate institutionalization, or to assist with appropriate placement if required.

Case Management Services for adults collaborates with individuals to access the supports and services they need to meet their basic needs and to provide guidance on their journey as they work towards meeting their personal goals.

Home and community-based services are provided to adults over 60 years of age, and to disabled adults over 18 years of age, in an effort to allow them to remain in their homes rather than move to institutional placements. Screening for adult care residences and nursing home placements are also provided. Eligible adults may receive up to 15 hours/week home based services, such as cleaning and meal preparations.

Sponsored Residential program offers compassionate care in a home-based setting by carefully matching children and adults with developmental disabilities to sponsored residential homes. Our sponsored residential home providers empower individuals with developmental disabilities to be active members of families, experience community involvement, and find normalcy in everyday life. Supportive Services for Veteran Families helps veterans and their families find and stay in stable, permanent housing. Focus on intensive case management to help resolve housing crises and ensure that they don't happen again.

ServiceSource currently offers 14 Long-Term Community Integration Services (LTCIS) programs in the City of Alexandria and Fairfax, Arlington and Prince William County, Virginia. Through person-centered supports, these programs maximize individual choice and community integration. In addition, each program participant has a uniquely designed plan that incorporates the individual’s skills and interests.

The program provides short-term case management support to attain permanent housing for Veterans experiencing homelessness. Veterans experiencing homelessness may call the screening line at 202-658-9599 to be assessed for services. Veterans should leave a message and it will be returned promptly. For Veterans who are not eligible for services through the Veterans First program, will be provided with referrals to appropriate community resources that may be able to provide further assistance. Provides energy assistance through EnergyShare program.

Horizon's case management services are provided to support individuals with a mental health illness, substance use disorder or intellectual and developmental disabilities. Case Managers: Understand that physical health and environmental stressors can affect a person’s overall well-being. Coordinate with local resources and organizations when necessary to address whole-person health consistent with a wrap-around care model. Strive to connect individuals and families to community resources, needed services, and monitor for satisfaction of services and support received.
