The Regional Post-Adoption Consortium Services (RPACS) provides basic and enhanced post-adoption services to adoptive families with children or youth under the age of 18 who reside in the Central and Eastern regions of the state at no cost. VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.

The case managers coordinate all services offered by the various providers to children in need, and actively seek input from families and medical practitioners to achieve the best possible results. Each Care Connection for Children program offers: coordination, information, education, social support and referrals.

Provides a network of therapeutic foster homes for children 8 to 18 years of age. This treatment foster care program is for children in the community who don't need to be in a group home, but need more attention and support than is provided by traditional foster care. Youth and families benefit from extra support and supervision. The program is able to accept emergency placements and to provide long term foster care leading to permanency. Provides an assessment, via home visit, to determine the needs that a senior citizen may have. Assists clients in obtaining services such as home-delivered meals, in-home services, transportation and options counseling. Includes the development of a care plan and monitoring of services once they are in place.

The Child & Family Network Centers (CFNC) support all parents in our program by building trusting relationships with them through one-to-one meetings three times per year. We offer case management, home visits, navigation to basic needs services, and a variety of workshops provided on-site to further empower our families. Mental health case management works to ensure that service systems and community supports are responsive to the changing needs of adults with serious mental illness. This service seeks to assist individuals in gaining access to needed medical, psychiatric, housing, financial, social, educational, vocational, and other supports essential to meeting basic needs. The individual is a partner in assessing their own needs, obtaining services and other supports, monitoring those services, and in preventing and managing crises. Case management utilizes a person-centered approach with a focus on promoting health and recovery.

Offers services to older adults and/or individuals over 18 with disabilities. Services include counseling; Medicaid pre-admission screening for nursing homes, adult care residences and in-home personal care; housing alternatives counseling; home-based care advocacy; companion services; and liaison with community resources and agencies. Transportation provided only under limited conditions.

Provides case management for elderly Medicaid recipients. Services include assessment, development of a care plan, assistance with enrollment in public benefit programs, counseling and coordination of services. Works with families and other care providers to assure the provision of needed services which will allow elderly individuals to remain independent and in their own homes.

Person-centered Aging Services Coordination is an interactive decision support process where people are supported in making long-term support choices that reflect their unique preferences, strengths, needs, values, and circumstances. Aging Services Coordination is provided by individuals trained and practicing according to Virginia's Statewide Standards.
