Free ongoing support for families of children/youth with special needs. Help families to coordinate care and to connect with the medical and community-based services specific to each child’s individual needs, including special education. We offer information and resources to anyone seeking information about children with disabilities (enrollment not required).

Provides decision support whereby individuals, with support from family members, caregivers, and/or significant others, are supported in their deliberations to make informed long-term support choices in the context of the individual's preferences, strengths, needs, values and individual circumstances. MEOC is a No Wrong Door agency.

Home and community-based services that allow the elderly or adults with disabilities to remain in their homes rather than move to institutional placements. Screening for adult care residence and nursing facility placements is provided.

Assists individuals who are 60 years of age or older, have dependencies in two or more activities of daily living (bathing, dressing, toileting, feeding) and have two or more unmet needs, remain in their homes.

Coordination of resources and support designed to facilitate independence and stability for adults with intellectual/developmental disability. Case management activities include assessment, planning, linking, and monitoring services; crisis prevention and intervention services; and hospital and state facility admission and discharge planning activities.

Care Transitions is designed to help reduce avoidable readmissions to hospital, by empowering individuals to manage their own health. A Care Transitions Coach will work to transfer skills that will allow an individual to recognize red flags, communicate effectively with their primary care physician, identify medication errors, and keep track of important medical information, with the goal of significantly reducing the likelihood of an unnecessary return to the hospital within 30 days.

Operate the No Limits day support club and provide information and referral services to survivors of acquired brain injury.

Adult Services are designed to allow the adult to remain in the least restrictive setting and function as independently as possible. Services are provided to impaired adults age 18 or older. Services may include the provision of home-based care, transportation, adult day care, placement in an assisted living or nursing home using the screening process and referral to other supportive services.

The Youth and Family Support Services Program assists families with youth between the ages of 3-17 experiencing challenging peer and family relationships and life crises by strengthening parental and family relationships through prevention, education, and supportive case management services.

The Family Partner Program works together for families in Southwest Virginia to help support each individual family in service development according to their unique needs. Advocates for your family. Also affiliated with the local chapter of the Federation of Families for Children's Mental Health.

Care Coordinators answer questions, perform assessments, find and coordinate resources, help apply for benefits, and more. Think of them as the liaison for older adults in the care network focused on addressing the needs of older adults and caregivers.

My Path Forward Legacy provides housing and wrap-around services for youth who have aged out of foster-care, equipping them with skills to achieve independence and personal success. My Path Forward Independent Living provides Independent Living Arrangement Services for youth 17-20 year of age.

Services provided include: information and referral, assessment, long-term case management, independent living skills training, job training/finding/coaching, transportation, respite care, socialization and recreation, mental health and behavioral services, day activity programs, and supervised independent living. Clients of the YWCA's Early Childhood and/or Domestic and Sexual Violence Services are automatically eligible for supportive services. Community-based Counseling clients or others experiencing sexual and/or domestic violence may contact us to receive services and/or referrals to appropriate resources. Those in crisis may call the 24-hour Richmond Regional Hotline, a confidential and free resource for survivors and their advocates.

The Healthy Start Loving Steps program provides case management services to reduce infant deaths. The Loving Steps provides nutrition counseling and case management services during pregnancy and after the baby is born. Also serve families that have an infant or toddler with nutrition related medical problems.

Services offered to individuals who meet eligibility requirements. Preadmission screening for nursing home placement and Adult Care Residence placement. Assessment for home-based care and referrals for other services.

Serves at-risk low income children ages 0-6 and their families. Case Management is a service that is responsive and flexible to the needs of each child and family. Each child and his/her family are assisted in gaining timely access to needed medical, psychiatric, therapeutic, social, educational and other supports. The core of the case management process is teamwork and trust. Case Managers work in partnership with families in order to develop goals that will enhance a child’s potential and quality of life in the community. Case managers assess the child’s needs, develop plans to meet those needs, help the family find services near where they live, navigate school and educational services, monitor the child’s progress, provide follow-up with family to ensure needs are met, and advocate for services and supports.

Case management team works with survivors to develop a person-centered plan for success and work with individuals to help locate and access supports in the community.

ElderLink provides affordable care management to older adults in Northern Virginia. Services include: An in-home assessment to determine what type of care is needed; Development of a comprehensive care plan; Assistance in finding and obtaining services; Monitoring to ensure appropriate care.

Serves senior citizens through programs and services enabling the individual to remain independent and productive. Also participates with the Hampton Roads chapter Alzheimer's Association for subsidized in-home respite care. Agency provides in home care, care coordination, transportation, congregate and home delivered meals.
