Baby Care: Special assistance to access community services for Medicaid eligible infants aged two years and under that are at risk for poor health. Promotes healthy outcomes for at risk pregnant women and infants through home visiting and education which is provided by registered nurses.

The Healthy Start Loving Steps program provides case management services to reduce infant deaths. The Loving Steps provides nutrition counseling and case management services during pregnancy and after the baby is born. Also serve families that have an infant or toddler with nutrition related medical problems.

Provides a network of therapeutic foster homes for children 8 to 18 years of age. This treatment foster care program is for children in the community who don't need to be in a group home, but need more attention and support than is provided by traditional foster care. Develops a plan of care, in home assistance arrangements, meals on wheels, assistance with complex paperwork from other agencies, Alzheimer's and Dementia support groups, adult day care, home repair and weatherization, health insurance counseling, legal aid, friendly visitor, telephone reassurance, and transportation.

MVLE Support Services provides therapeutic activities both on-site and in the community for individuals served. The goal of this program is to provide community inclusion, community education, and a path toward greater independence. Services are offered at all three of MVLE's Community Centers located in Springfield and Chantilly, VA.

Care Coordinators answer questions, perform assessments, find and coordinate resources, help apply for benefits, and more. Think of them as the liaison for older adults in the care network focused on addressing the needs of older adults and caregivers.

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.

Provides planned combinations of individualized activities, supports, training, supervision and transportation to individuals with intellectual disabilities to improve their skills or maintain an optimal level of functioning. The program is designed to provide training and support in non-vocational areas such as self care, independent living, communication, socialization and motor skills.

Adult Services are provided to adults age 18 and older who have a disability and to their families when appropriate. Adult Services may include the provision of case management, home - based care, transportation, adult day care, nutrition services, placement services, long term care pre-screening for Medicaid-funded services, guardianship monitoring, and other activities to aid the adult.

Family Care homes provide a person-centered, family-based home in which an individual with developmental disabilities can become an active member of a family. At the time of the initial assessment, a Plan of Care will be formulated, and all clients will be reassessed annually or as their conditions/needs change.
