Assists low-income families with children ages and 6 and under and pregnant women with access to medical and dental services and acts as a link to other human service organizations/providers in the NRV. Offers comprehensive case management in the home where children receive consistent and comprehensive care in a familiar setting. CHIP services, provided by a nurse and a home visitor team, include helping families make and keep appointments and follow physician recommendations, educational home visits, parenting education, helping families set and reach personal and family goals, transportation to medical and dental appointments, medical case management, and referrals to other community-based services to help meet the multifaceted needs of families. All services emphasize prevention and early intervention. This is not the state CHIP insurance, but will help families access Medicaid or FAMIS (state CHIP insurance). Not a substance abuse/ mental health provider but can connect families to those resources. Transports only if no other means available.

Care Transitions is designed to help reduce avoidable readmissions to hospital, by empowering individuals to manage their own health. A Care Transitions Coach will work to transfer skills that will allow an individual to recognize red flags, communicate effectively with their primary care physician, identify medication errors, and keep track of important medical information, with the goal of significantly reducing the likelihood of an unnecessary return to the hospital within 30 days.

Case management and care coordination services with independent, private specialists and practitioners regardless of a family's income. Provides a "pool of funds" for uninsured and underinsured children with special health care needs. The CHKD Care Connection for Children pulls together case managers, medical personnel, educators and family members to determine how children with special health care needs - those with chronic physical disorders that are expected to last for at least 12 months - can reach their maximum potential.

Provides women facing unplanned pregnancies with resources, referrals, adoption services and information allowing them to make good decisions regarding the health and well-being of both themselves and their unborn child. Receive answers to questions and receive support in the decision making process. Locates available resources in the community that may be able to assist with medical care and related expenses. Works with both parents, either separately or together, in planning for the baby's needs. Services include: *Free counseling for pregnant women; *Adoption planning; *Referral services; *Follow up and support; *Baby items.
Provides programs to children ages 6-18 and their families who live in Newport News. MVLE Support Services provides therapeutic activities both on-site and in the community for individuals served. The goal of this program is to provide community inclusion, community education, and a path toward greater independence. Services are offered at all three of MVLE's Community Centers located in Springfield and Chantilly, VA.
Serves children birth to age 21 with special health care needs. Provides care coordination including referrals to health care services, case management, and referrals community support and resources. Maintains a database of regional and local resources for children with special health care needs. Financial assistance may be available to assist with specialty medical care, if income and medical criteria are met. Also loaner hearing aids for children are available through the Virginia Hearing Aid Loaner Bank housed at UVA. The Care Connection for Children (formerly known as Children's Specialty Services) program is divided into 6 Centers of Excellence throughout the state. The Charlottesville area center is known as the Blue Ridge Care Connection for Children (BRCCC). It is a collaboration between the Virginia Department of Health and the University of Virginia Children's Medical Center. The BRCCC is directly affiliated with the UVA Children's Medical Center. BRCC serves the Counties of Albemarle, Augusta, Clarke, Culpeper, Fauquier, Frederick, Greene, Highland, Madison, Nelson, Orange, Page, Rappahannock, Rockingham, Shenandoah, Warren and Cities of Charlottesville, Harrisonburg, Staunton, Waynesboro, Winchester There are 5 other centers across the Commonwealth. Early Intervention for Children With Disabilities/Delays
Assists adults with brain injuries and their families in developing a plan and obtaining services for maximizing independence and quality of life. Services provided include: information and referral, assessment, long-term case management, independent living skills training, job training/finding/coaching, transportation, respite care, socialization and recreation, mental health and behavioral services, day activity programs, and supervised independent living. Call ahead for sign language.
The BabyCare program is a home-based program that fosters safe and healthy pregnancies and healthy babies by providing free education, care coordination, and support services from registered nurses to high-risk pregnant people and infants up to 24 months of age. Services Include: - intensive case management - coordination of care - Pack 'N Plays - education about prenatal care, birthing, infant care, parenting and community resources. Free child safety seats and booster seats are also available.

Home and community-based services that allow the elderly or adults with disabilities to remain in their homes rather than move to institutional placements. Screening for adult care residence and nursing facility placements is provided.
VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.
CBIS, Case Management Program is designed to meet the long term case management needs of adult survivors of brain injury in the Metro Richmond area. Through the case management process, clients are empowered to create their own individualized goals and improve their ability to make decisions and increase their independence. CBIS case management services is part of Virginia's safety net service system for persons with brain injury.
