Provides planned combinations of individualized activities, supports, training, supervision and transportation to individuals with intellectual disabilities to improve their skills or maintain an optimal level of functioning. The program is designed to provide training and support in non-vocational areas such as self care, independent living, communication, socialization and motor skills.

Links older people with appropriate long-term care services, so they can maintain their independence. Care coordinators provide outreach, assessment, and care planning. Through this program, individuals learn about and/or receive assistance from AASC and partner organizations. Here are just a few of Care Coordination's services: Medicare Part D enrollment; Insurance counseling (VICAP); Information and assistance Farm Market Fresh; Tele-N-Touch telephone reassurance; Emergency services; Disease prevention and health promotion; Fan care; Heating and cooling assistance; Medication management; My Medicare Matters computer training, and Centenarian birthday recognition.
Adult Services are provided to adults age 18 and older who have a disability and to their families when appropriate. Adult Services may include the provision of case management, home - based care, transportation, adult day care, nutrition services, placement services, long term care pre-screening for Medicaid-funded services, guardianship monitoring, and other activities to aid the adult.
Adult Services include the provision of companion services to the elderly and screening to determine the need for possible nursing home or adult home placements.
The Community Support Program offers Loudoun County residents short-term case management and assistance to meet basic needs, including support and resources for prescriptions, food, rent, and utilities.

VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.

Professional Case Managers partner with families to coordinate care and help find the best healthcare and community resources in Northern Virginia. Education consultant helps famlies to communicate wit the school about special education. Parent support coordinator provides parent education and family support. Limited funds available to assist children who are uninsured or underinsured to receive care they otherwise could not afford. Good Shepherdof Northern Virginia provide appropriate resources to prevent or address homelessness and teach life skills required to help individuals and families return to stability for long term success. The Six Steps to Self-Sufficiency program is an intensive case management approach to goal-setting for long term achievement of stability for households.
Provide case management to coordinate needed services such as medical, mental health, substance use services, housing, employment, benefits acquisition. Assist people enrolling for Medicaid.
Project LINK offers services to women and children whose lives have been negatively affected by substance use. This may include the woman’s own substance use, her partner’s use, or use by a family member. Priority services are given to women who are currently pregnant, parenting, or seeking to regain custody. The program provides comprehensive, individualized, family-focused, gender specific case management services.

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to : assisting in placement such as in an adult home, nursing homes, etc.; transportation when needed; companion services designed to assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization.

Goodwin Living at Home members are able to stay in their homes as they age, while being given support for all stages of life and at all levels of living.
Healthcare and benefits assistance for recently discharged veterans with service in a theater of combat operations.
Care Coordinators answer questions, perform assessments, find and coordinate resources, help apply for benefits, and more. Think of them as the liaison for older adults in the care network focused on addressing the needs of older adults and caregivers.
Provides a program where case workers set up plans for continued medical care for patients on leaving the hospital. Provides crisis intervention, information and referral.
Helps young clients and their families to access available resources. Case managers provide assessments, long-term treatment planning, coordination of services, client advocacy, support outreach, and crisis prevention. Limited client transportation is available.

Professionally certified care managers assess older person's living situations and provide cost effective options to meet their needs. Client fees based on ability to pay. Develops a plan of care, in home assistance arrangements, meals on wheels, assistance with complex paperwork from other agencies, Alzheimer's and Dementia support groups, adult day care, home repair and weatherization, health insurance counseling, legal aid, friendly visitor, telephone reassurance, and transportation.
