Encourages and facilitates participation in individualized activities, ensures safety, and provides needed practical support such as the administration of medication, transportation to community outings, etc. An overall focus on maximizing potential and skill-building is at the core of day support values. Serves adult men & women with the primary diagnosis of Intellectual Disability (diagnosis must be documented as originating prior to age 18). Provide care, assistance and supervision to individuals in their own homes. These services are supplemental to the care provided by a primary caregiver or they may support an individual whose level of independence does not require a primary care provider. Each resident has a plan of care outlining support needs. This plan outlines specific outcomes expressed by the individual or evaluated as an area of need/challenge. The plan is used to help each individual progressively strengthen his/her own individual knowledge, skills and independence.
