Horizon Center is a Psychosocial Rehabilitation Program (PSR) and day treatment program providing rehabilitative services for individuals with mental health and/or developmental disability concerns. Horizon PSR program provides an environment where individuals can develop or regain interpersonal and daily living skills, develop a supportive system of peers and providers, and benefit from a supportive structure.

Adult Day Care - Haysi, Generations at Haysi, Appalachian Agency for Senior Citizens

Bridges the gap between children and older adults in a safe, homelike setting. Provides preschool and before and after school programs, all in an intergenerational atmosphere. Together, children and adults have participated in special activities, including a pen pal program and a celebration of our Appalachian heritage. Integrates the wisdom and insight of the elderly with the energy and enthusiasm of the young. Intergenerational day care offers children the opportunity to take part in carefully selected, supervised activities with senior citizens. It also allows family members to work or take a break from caring for a loved one. Skilled staff create individual care plans for participants. Staff includes a supervisor, activities director, registered nurse, and personal care attendants. Participants receive recreation, socialization, health monitoring and medication management in a home like environment. Each day we serve a USDA-approved breakfast, lunch, and afternoon snack. Collaborate with the AllCARE for Seniors PACE program. Through this partnership, participants receive socialization, in conjunction with therapy and medical services while they attend day care. Generations continues to offer training and support sessions to caregivers throughout our service area.

These programs serve people who are recovering from mental illness by providing a safe and supportive environment that promotes independent living, social and interpersonal skills, and relapse prevention. Attending daily classes provides education about mental illness, substance use, and appropriate medication. Engaging in volunteering opportunities and engaging in social activities promotes empowerment, recovery, and competency. Services include: Face-to-face Service Specific Provider Intake (SSPI) that identifies treatment needs Development of Individualized Service Plans (ISP) which identify an individual's needs and goals in the area of mental health recovery and stability in the community Psycho-educational activities to teach about mental illness and appropriate medication to avoid complications and relapse Social skills training, peer support, and community resource development to promote empowerment, recovery, and competency Opportunities to learn and use independent living skills, and to enhance social and interpersonal skills Field trips to provide training in an integrated setting in order to increase an individual's understanding or ability to access community resources and meet goals outlined in the ISP

Adult Day Centers - Leesburg - Loudoun County Area Agency on Aging

Loudoun County Adult Day Centers help aging adults remain in the community as long as possible, providing support and respite to caregivers. All Centers are licensed by the Virginia Department of Social Services. Three licensed adult day centers provide therapeutic activities for older adults who cannot safely stay home alone and who would benefit from a supportive environment.
