Offers care to seniors in their home or institutional facility. Home Instead Senior Care is a non-medical companionship and home care services for seniors in their homes and facilities. A licensed provider of Personal care services. A wide variety of services include: meal prep, medication reminders, light housekeeping, laundry, and incidental transportation to doctor appointments, errands and shopping. Direct one on one services are individually planned to meet each client's unique needs and preferences. Services are provided from a few hours to 24/7, seven days a week including overnights and weekends. Staff is available 24/7 to answer all questions.

Home-based care provides personal aide and hygiene care, light housekeeping, supervision and other supportive services to elderly and disabled adults to the extent that funds are available, so that they may remain in the community and out of institutional placement. Adults must meet all eligibility criteria to qualify and there may be a waiting list for service.

JZC Community Homes LLC provides supportive residential living services for adults with intellectual disabilities (ID) and developmental disabilities (DD) in a safe, stable, and person-centered home environment. Residential services take place in a licensed or DBHDS-authorized residential home. The goal is to help each individual build independence, strengthen daily living skills, maintain their health and well-being, and thrive within their home and community.

Serves adult men & women with the primary diagnosis of Intellectual Disability (diagnosis must be documented as originating prior to age 18). Provide care, assistance and supervision to individuals in their own homes. These services are supplemental to the care provided by a primary caregiver or they may support an individual whose level of independence does not require a primary care provider. Each resident has a plan of care outlining support needs. This plan outlines specific outcomes expressed by the individual or evaluated as an area of need/challenge. The plan is used to help each individual progressively strengthen his/her own individual knowledge, skills and independence.

CVACL's Personal Care Services provides assistance for older adults who are unable to perform daily living activities such as eating, dressing, bathing, toileting, grooming, transferring in and out of a bed or chair, or walking.

A private agency licensed by the Department of Behavioral Health and Developmental Services (DBHDS) providing In Home Support Services to individuals with an Intellectual and/or Developmental Disability (IDD). Horizon’s ICF programs offer adults with a primary diagnosis of intellectual disability with active treatment in functional skills to create an environment in which the individual can not only live safely but can excel. Horizon’s ICFs also support individuals with a variety of medical and behavioral needs that require extensive supports to be healthy. High quality, comprehensive services are provided by Horizon's interdisciplinary team of ICF staff which includes doctors, nurses, psychologists, dieticians, recreational therapists, OT/PT speech therapists, residential technicians, counselors, family social workers, case managers, and citizen volunteers among others. All work together to monitor Service Treatment Plans, medical needs, and provide daily living support which includes full-service laundry, cooking, cleaning, grocery shopping and more. The result is the ability to provide continuous, direct care twenty-four (24) hours per day, with 24/7 oversight and on-call availability after normal business hours.

Supplement for aged, individuals who are blind or disabled in Homes for Adults who have insufficient funds to meet their needs. An AG recipient must reside in a licensed adult home and both financial and non-financial eligibility standards must be met. An AG recipient is automatically eligible for full coverage Medicaid.
