Health Education offers education information, referral, and resource assistance for a wide variety of health issues. Education materials are provided in health fairs, wellness screenings, community locations, and for groups requesting specific topics.

Responds to needs of the elderly populations in the Northern Shenandoah Valley Region through planning, coordinating and administering the following services: Insurance Counseling program, Senior Transportation, Senior Center Programs, Congregate and Home Delivered Meals, Cooling Assistance, Utility Assistance, Group Respite Day Care Programs, In-Home Personal Care and Homemaker Services, Case Management, Information and Referral, Long-term Care Ombudsman Program.

No Wrong Door is a virtual single point of entry for accessing public and private health and human supports for older adults and adults with disabilities. The vision of No Wrong Door in Virginia is to provide older adults and adults with disabilities with comprehensive information and streamlined access to available long term supports through collaborative partnerships and shared technology.

Provides basic health services for migrant and seasonal farmworkers and their families in Southwest Virginia. Offers many services on a sliding scale to migrant and seasonal farmworkers. Services include, but are not limited to, the following: primary care medical services, health education, outreach, health assessments, referrals, and case management. The MHN outreach workers identify health problems and assist farmworkers to obtain health care at one of our clinical partners centers.

The Community Health Education Center provide information on general health topics, cancer and women's and family health. CHEC welcomes anyone with questions on any health topic. CHEC has print books and magazines, internet access, video viewing and a children's area. CHEC has materials for all ages and all reading levels. CHEC staff can help users find need information.

Provides assisted living and is located in the beautiful Shenandoah Valley. Offers both private and semi-private rooms, married couples may share a room, room accommodations with private baths available, community dining area, special dietary restrictions honored, medications are available as needed, assistance available for bathing, dressing, eating, toileting, transferring, meal preparation, laundry and housekeeping. As we strive to maintain independence care is augmented by physicians and psychiatric and psychological assistance. We accept ambulatory, non-ambulatory and individuals affected by Alzheimer's disease or a related dementia.

Care Transitions is designed to help reduce avoidable readmissions to hospital, by empowering individuals to manage their own health. A Care Transitions Coach will work to transfer skills that will allow an individual to recognize red flags, communicate effectively with their primary care physician, identify medication errors, and keep track of important medical information, with the goal of significantly reducing the likelihood of an unnecessary return to the hospital within 30 days.

The J-Fit Health Club and Wellness programs offer a balance of programs that nurture the mind, body and spirit. The goal is to provide awareness of making choices toward a more fruitful existence related to the various aspects of one's life. Programs in the Health, Fitness & Aquatics Department meet the needs of individuals of all ages and levels of physical and mental ability.

Bradley Free Clinic refers eligible Bradley patients to local physicians who agree to provide free primary and specialty medical services to low income, uninsured residents of the Roanoke Valley and surrounding areas in their practice office
