"A Matter of Balance" is a fall prevention program designed to reduce fear of falling and encourage physical activity in older adults. It involves interactive workshops, exercise training, and environmental modifications to address the root causes of falls. The program is offered in small groups, led by trained coaches, and typically consists of eight two-hour sessions.

Services include health risk assessments; routine health screening; nutritional counseling and education; health promotion; physical fitness and group exercise; music and dance-movement therapy; home injury control; mental health screening; mental health education; information on Medicare coverage; medication management; information on diagnosis, prevention, treatment and rehabilitation of age related diseases and chronic disabling conditions geriatric counseling and follow up health services counseling. Services are available to consumers unable to leave their homes.

Provides structured medical case management and wrap-around services for children birth to seven who have been diagnosed with a respiratory disease or have asthma that is poorly controlled. These services are designed to complement the care an asthmatic child receives through his/her primary care provider or pulmonologist. Through monthly in home evaluations of the child, medical records and individual case consultation with a child's primary care provider or pulmonologist, nurses obtain information about a child's asthma profile including environmental triggers, medications, history of attacks, frequency of emergency visits and hospitalizations, and treatment recommendations. Instructs parents on how to make changes in the home to decrease environmental triggers (dust, pet dander, indoor smoking, mold), how to use and manage their child's medications (including proper dosage), and how to use spacers that make the most of asthma medications. Nurses also instruct parents with regards to managing an asthma attack including how to recognize the signs and symptoms of an impending attack, when to contact the child's primary care provider or pulmonologist, when to take the child in for evaluation, and when to take the child to the Emergency Room. Follow ups with the parent and child's doctor to ensure that treatment recommendations are understood and followed; families with transportation needs can receive transportation through CHIP to any of their child's health care or specialist appointments. Together with parents and physicians, nurses support the development of an Asthma Action Plan that provides clear, easy-to-follow instructions on managing an asthma attack.

Provides information and supportive services to parents in Wise, Lee, and Scott Counties and the City of Norton. All first-time parents qualify for a free Welcome Baby Package filled with pregnancy and/or parenting information, samples of baby products and a welcome gift. A voluntary program that provides a family with a home visitor that answers questions, calm fears, provide development and health information, support, and connect parents with resources located in their community. Assists parents in discovering ways to help their child be the very best that he or she can be. Holds parent "Get-togethers" that are educational and fun activities, a chance to meet other first-time parents, on-site childcare available if needed during the meeting. Transportation provided or arranged to doctor visits, parent "get-togethers," other important appointments.

Provides a wide range of services to assist the elderly and their families to improve or maintain the senior citizen's quality of life. Nurses provide health education, blood pressure screenings, and serve as advocates for the elderly. The nurses also provide pre-admission nursing home screenings for nursing home placement, respite, adult day care and personal care services for clients who are Medicaid eligible. Flu and pneumonia clinics are held annually in sites throughout the city and are provided to all citizens at a nominal fee.

Virginia Insurance Counseling and Assistance Program (VICAP) is a free counseling program for individuals aged 60 and older and their families. VICAP helps them to understand Medicare, Medicaid, medical Medigap and / or Long-Term Care insurance is also provided. Counseling can be provided by telephone or in person. Also, will assist eligible clients with filing insurance papers, understanding insurance policies and Medicare processes.

Hospice and Palliative Care benefits are available to persons who have been determined to be in the end stages of a terminal illness and are no longer seeking or receiving curative treatment of any kind. The goal of hospice and palliative care is to assist in providing "in home" management of physical and emotional problems associated with terminal illness such as the relief of pain and distressing symptoms. Services are directed toward maintaining a high quality of life for both patient and family/significant other using team support and guidance.
