Provides a program designed in accordance with the American Diabetes Association clinical practice recommendations. Its purpose is to increase access to education, reduce medical costs and prevent complications for people with diabetes. Initially, each participant meets with a diabetes educator to learn the basics of the program, receives a baseline assessment and sets personal goals. Group classes are available. Topics covered include nutrition, exercise, stress reduction and grocery shopping.

Inova Diabetes Center offers comprehensive diabetes education classes, programs and support groups.

The Diabetes Management education program is recognized by the American Diabetes Association offering education on a variety of topics including Type 1 and Type 2 diabetes, medical nutrition therapy, insulin pump initiation and support, gestational diabetes and pre-diabetes. Includes assessment with a Certified Diabetes Educator for a medical history, a brief physical exam, and goal setting for behavior changes. Follow-up care with a dietitian or RN for weight management, blood glucose control, and progress toward goals. Private instruction with an RN for insulin initiation, adjustments and management. Nutrition Counseling includes meal planning, carbohydrate counting, weight management, and incorporating meals and exercise. Day and evening classes are offered.

Offers a team approach to providing the skills needed to manage diabetes: nutrition, exercise planning, blood glucose monitoring and discussion of medications and their actions. Serves adults with Type 1 or Type 2 diabetes and children and pregnant women with diabetes. Provides diabetes support groups for adults and children and their parents.

Provides a diabetic foot clinic for those diagnosed with diabetes.

Provides a support group for diabetics that is open to all diabetics and their families. Offers an educational program, blood sugar testing and time for member discussions and sharing of experiences with regard to diabetes.

Individual instruction and case management in self-care and maintenance of diabetes. Diabetes nurse educator, nurse practitioner on site. Medication help available.

Identifies persons at risk for sensory loss/changes from diabetes and refers those at risk for appropriate medical care. Screenings are held at Health Fairs and at the Rockingham County Fair.

he EVMS Strelitz Diabetes Center provides education, in-patient and outpatient care, and research focused on minimizing complications of diabetes and finding a cure.

Offers care for chronic conditions by appointment only. Conditions include Diabetes, Asthma, GERD, Hypertension, and other chronic diseases.

Educates people with diabetes about issues relevant to their health. Different topics are addressed by presenters who are specialists in their fields. Provides free medical care to the indigent in Warren County. Day Clinic is for the treatment of chronic illness including Diabetes, Hypertension, Heart Disease and other ongoing illness by appointment only Mondays and Wednesdays. A walk-in clinic for existing patients on Tuesdays. We also offer an Every women's Life Clinic. Centra's Diabetes program is designed to promote diabetes self-management. We provide diabetes education to patients, family members and caretakers, and we offer inpatient as well as outpatient education. All we ask is that patients provide a physician referral so we'll have information about their needs, and their medical providers will know the proper instructions. Through our program, people recently diagnosed with diabetes learn survival skills including: A basic intro to the disease process Foot care Eye and dental health Meal planning and food label reading Guidelines for sick day management Medication information (oral or insulin) Self-monitoring guidelines Instructions on use of the glucose meter Exercise to promote better control of the disease and improve well-being Managing complications Traveling skills Intensive insulin management Hotline available to answer diabetic related questions. Monthly meetings are held with informative talks about diabetes. Certified Diabetes Educators, Clinical Dietitians, Pharmacies and other staff members at Augusta Health are also available to speak with individuals who have been diagnosed with diabetes and their families. The group is free and open to the public. No registration is required.
