Monthly meetings are held with informative talks about diabetes. Certified Diabetes Educators, Clinical Dietitians, Pharmacies and other staff members at Augusta Health are also available to speak with individuals who have been diagnosed with diabetes and their families. The group is free and open to the public. No registration is required.

This program provides a timely screening assessment for Medicaid or Medicaid eligible clients needing nursing home placement or services in the home or community. The target population for this program includes the elderly, disabled adults, or children needing a waiver for long-term placement or services for home-based care. Screenings are conducted in partnership with the Department of Social Services (DSS) and requests for screenings need to be made through DSS. A nurse and a social worker screen the client to determine if they meet Medicaid's criteria for either (1) Functional capacity: the degree of assistance an individual needs to complete the activities of daily living (bathing, toileting, dressing) or (2) Medical or Nursing need (wound care). Services may include the following: in-home care, adult day care, or admission to a care facility. Referrals for screenings may be made through your local Health Department.

Part C Early Intervention provides a full range of services for infants and toddlers with developmental delays or disabilities and their families. Services aim to provide formal and informal support to meet the outcomes identified by families for their children. Provide outreach, coordinate services, and make referrals in the community as needed.

Provides non-emergency medical transportation for Medicaid recipients who have no other means of transportation and need to go to a physician or a health care facility.

In limited areas VPAS' can provide transportation to medical appointments, grocery store, pharmacy, and bank when no other transportation options are available. Riders must pre-register prior to transportation being provided. Advance notice is required

Provides assisted living and is located in the beautiful Shenandoah Valley. Offers both private and semi-private rooms, married couples may share a room, room accommodations with private baths available, community dining area, special dietary restrictions honored, medications are available as needed, assistance available for bathing, dressing, eating, toileting, transferring, meal preparation, laundry and housekeeping. As we strive to maintain independence care is augmented by physicians and psychiatric and psychological assistance. We accept ambulatory, non-ambulatory and individuals affected by Alzheimer's disease or a related dementia.

Areas Served: Harrisonburg City, Rockingham County. Provides services for eligible children and their families, with developmental delays and disabilities. Services include, but are not limited to, speech therapy, physical therapy, occupational therapy, developmental services, audiology, and vision.

Provides speech/language services to all ages. Treats communication problems (e.g., language, articulation, fluency, voice, etc.). Evaluates and treats hearing disorders. Additional semester fees apply for speech-language therapy or aural rehabilitation, including hearing aid services. Demonstrates and evaluates assistive listening devices. The Blood Cancer United Family Support Groups program gives patients and their families a place to go where they can share information, education and feelings in a comfortable and caring environment.

RAMPS partners with high school student volunteers to provide wheelchair ramps at no cost to low-income, elderly or disabled individuals residing in Henrico, Hanover, Chesterfield Counties, and the City of Richmond.

Promotes the creation of social, recreational, and vocational programs designed to meet needs of individuals with autism. Currently, activities offered include monthly socials, a day camp over spring break, and Camp Gonnawannagoagain, a summer day camp experience for children and teens with autism.

Area Served: City of Virginia Beach. Serves children from birth through age three who have developmental delays, atypical behavior, and/or a disabling condition that is likely to result in a delay. Children referred for services are reviewed by an eligibility team to determine their eligibility for services. If it is determined the child would benefit from services (i.e., is eligible) he/she will receive a full developmental assessment prior to beginning services. Services available to eligible children and families include, but are not limited to, service coordination; speech, physical, and/or occupational therapy; and educational services. Services are provided in the child's natural environment (e.g., the home, daycare, etc.) and are provided on a sliding fee scale. The program also offers center based classes and community based group activities for children.

Provides information, referrals and support for family members and others affected by autism. Offers monthly meetings, website with monthly newsletter, social activities, and seminars for those who are newly diagnosed.

DBVI provides services and resources which empower individuals who are blind, vision impaired or deafblind to achieve their desired levels of employment, education, and personal independence. Services are provided through 4 main programs: Vocational Rehabilitation, Rehabilitation Teaching and Independent Living, Low Vision Services and Education Services. Most services are provided in the person's home, school, or workplace. Referrals are accepted from individuals, families, schools, doctors, and other community partners.

Medicaid clients or persons who anticipate becoming Medicaid eligible are screened upon request to determine if significant needs exist which require admission to a nursing home. Based on the results of the screening, clients and families may be offered the option of nursing home placement or in home personal care services.

Provides non-emergency ambulance and wheelchair van transportation.

Veteran Directed Care serves eligible veterans of any age who are at risk of nursing home placement. Our services help veterans continue to live in their homes and communities for as long as possible.
