Carriage Hill has an Alzheimer's Program which is a separate unit within the location that is designed specifically for people with Alzheimer's and other memory-impairment disorders. The Comprehensive Alzheimer's program provides the full range of care and services for seniors with mid-to-early-moderate cognitive and functional impairment, and for those with moderate-to-early-severe impairment.
This program provides a timely screening assessment for Medicaid or Medicaid eligible clients needing nursing home placement or services in the home or community. The target population for this program includes the elderly, disabled adults, or children needing a waiver for long-term placement or services for home-based care. Screenings are conducted in partnership with the Department of Social Services (DSS) and requests for screenings need to be made through DSS. A nurse and a social worker screen the client to determine if they meet Medicaid's criteria for either (1) Functional capacity: the degree of assistance an individual needs to complete the activities of daily living (bathing, toileting, dressing) or (2) Medical or Nursing need (wound care). Services may include the following: in-home care, adult day care, or admission to a care facility. Referrals for screenings may be made through your local Health Department.
Offers a Developmental Day Support Program to individuals who need additional assistance in developing independence in the activities of daily living. The target population for the program will be those individuals with developmental disabilities who fall within the severe to profound range of Intellectual Disability. Provides public transit for the City of Galax. This service includes shopping routes, and transportation to congregate nutrition sites operated by District Three Senior Services, and town transit in Galax.
Vehicles are provided to transport seniors citizens to and from nutrition sites, agencies, recreational activities, and medical appointments.
Provides several weight management programs: Weigh 2 Go 12 weekly one-hour sessions. In addition to group discussion there is a private weigh-in and an opportunity to have food and activity records reviewed by the facilitator. Habits Not Diets 12 weekly one-hour sessions for weight management that focus on identifying and changing current eating habits that lead to weight gain. Performed in home by a public health nurse and adult services social worker for patients needing assessment of care level needed and as a prerequisite of nursing home placement. Consumers can choose to receive community based services rather than nursing facility based services if they meet the eligibility after the screening. The Medicaid Waiver programs may pay for community based services.

Psychological Services Center (PSC) is a comprehensive community based counseling center. We provide a full range of individual and family counseling services geared towards providing affordable and professional mental health care for children and adolescents. We also provide state of the art assessments and treatments for children, adolescents and adults that may have attention deficit hyperactivity disorder, conduct disorder, learning disorder, autism. We provide both short- and long-term therapy on an outpatient basis. The Clinic provides Assessment through diagnostic and psychological evaluations for infants/toddlers, children, adolescents, and adults who are suspected to have an Autism Spectrum Disorder, including Autism, Asperger Syndrome, and Pervasive Developmental Disorder NOS.

Provides an international service oriented civic club with emphasis on sight/hearing conservation. Provides assistive devices for hearing or sight impaired individuals who meet financial eligibility criteria. Also helps the needy obtain eye examinations.
