The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. Provides a long-term and rehabilitative care facility affiliated with Fauquier Hospital. Poplar Hill Health and Rehab, offers the community a unique continuity of care. The same physicians and therapists who serve at the Fauquier hospital also serve patients at the Poplar Hill Health and Rehab. Many patients recovering from surgical procedures such as total hip or knee joint replacement, or conditions such as a stroke, are placed at Poplar Hill Health and Rehab after hospitalization for several weeks of rehabilitation before going home. Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.
