Rehabilitative program on our transitional unit with the help of therapists and caring, long-term stable administrative and nurse staffing. This program is ideal for short-term, post-acute rehabilitation after a hospital stay. Our programs provide the same intensity of therapy services as you would receive in a hospital setting, while remaining close to home and family. The following are just some of the patient complications we serve in an inpatient setting: fractures/orthopedic recovery, elective joint replacements, strokes, automobile accidents, and heart surgery.

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.

The Rehab Center at Richfield provides short-term, skilled nursing and rehabilitation services to patients who are still recovering from surgery, injury or illness and serves as a bridge between a hospital stay and the patient's return home. The Rehab Center offers all private rooms certified for Medicare Part A and private insurance only. The Rehab Center and the Recovery & Care Center has assembled a team of caring, knowledgeable healthcare professionals. Services are coordinated and directed by a leadership team that includes the Administrator, Assistant Administrator and Director of Nursing. Provides a long-term and rehabilitative care facility affiliated with Fauquier Hospital. Poplar Hill Health and Rehab, offers the community a unique continuity of care. The same physicians and therapists who serve at the Fauquier hospital also serve patients at the Poplar Hill Health and Rehab. Many patients recovering from surgical procedures such as total hip or knee joint replacement, or conditions such as a stroke, are placed at Poplar Hill Health and Rehab after hospitalization for several weeks of rehabilitation before going home. A pool is on site for aqua therapy. Also offers chemotherapy and oncology services, hospice care and modern services for the medically complex patient. Also offers Physical Medicine and Rehabilitation (PM&R) services which include physical, occupational, and speech and language therapy for all ages. The primary goal is to return patients to an active, healthy lifestyle.
