CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver.
