PATHS Community Medical Centers provide comprehensive primary health care to those local to the Dan River Region. Services Include: Family Medicine - Primary care for the whole family including preventive services and annual screenings for children, adults and seniors; Internal Medicine - Primary care for adults including preventive services and annual screenings; Pediatric Medicine - Primary care for babies and children including well-child visits and school/sports physicals; Chronic Disease Management - Diagnosis and ongoing care for diabetes, hypertension, and asthma. Resource Centers are health clinics with a variety of services available in public housing communities. All services are offered to anyone, and cost is based on a financial eligibility sliding sale. The Resource Centers also refer and connect patients to primary care providers and other community services, empowering residents to get connected to the care they need.

Services provided include: adult and geriatric medicine; pediatric care; adolescent care; behavioral health; pharmacy; laboratory; minor surgery; emergency medicine; telemedicine; hospitalization; Mount Rogers Medication Assistance Program(MAP).

The Free Clinic of Franklin County provides health care services and medications to eligible low-income adult residents of Franklin County, or adults with Medicaid. Bernard Healthcare Center serves some under-insured adults, and provides community outreach services such as school sports physicals, PPD testing, glucose testing, and blood pressure checks.

The Ambulatory Care Clinic (ACC) is located off-site from Sentara Norfolk General Hospital. The ACC offers lab services, X-rays, and EKG's are also available for diagnostic work-up. A Medication Assistance Program is available to help patients get prescription medications free or at a reduced cost. External referrals for specialty care are also made to other Sentara providers in the community.

Prenatal care offered at this location up to 32 weeks. Accept most insurances, including Medicare, Medicaid and HMOs, and offer all services on a sliding scale based on family size and income.

The Free Clinic of Pulaski County serves those in the community who are uninsured, has Medicaid or Medicare Part A. and whose household income falls at or below 300% of the Federal Poverty Guideline. The clinic also assists patients in accessing the Medication Assistance Programs of the various pharmaceutical manufacturers.
