VPAS' Emergency Services program provides one-time immediate financial assistance and other resources, including referrals to other public and private agencies to older adults who have an emergency need. Emergency Services may include payment assistance for medication refills, assistive devices, and utility and rent payments. Assistance is provided one-time, per year, per older adult age 60+, who must have an emergency need for help which endangers their health and/or well-being.

Resource Centers are health clinics with a variety of services available in public housing communities, Broad Rock Community Center, and Southwood Apartments. All services are offered to anyone and cost is based on a financial eligibility sliding scale. The Resource Centers also refer and connect patients to primary care providers and other community services, empowering residents to get connected to the care they need.

Services offered: * Medical services provided by Licensed Physicians and Nurse Practitioners. * Lab services on site. * Full range of X-ray services available through referral. * Counseling services offered two days per week by Licensed Clinical Social Worker. * Black Lung services available through referral. * Sliding Fee Scale available to patients with no insurance and who qualify. * Free medication program available to patients with no insurance and who qualify. * Mother and Infant Care Center(MICC) program available.

Nonprofit community health center that offers affordable and comprehensive medical, dental, behavioral health and pharmacy services (services vary by location). Accept most insurances, including Medicare, Medicaid and HMOs, and offer all services on a sliding scale based on family size and income.

Available to assist families completing applications to provide health insurance for their children.

Enrolls patients who qualify in pharmaceutical donation programs. Not all medications are available. Each company requires eligibility determination for medication requested (in addition to meeting clinic eligibility). No scheduled drugs/narcotics.

The Community Pharmacy, a separate program administered by the Clinic, provides low-cost prescription medications to individuals with household incomes up to 300% of the federal poverty guidelines. These individuals are patients at private physicians' offices or other health care facilities and receive pharmacy-only services at the Clinic.

Provides county residents with comprehensive preventive health care that includes a broad range of diagnostic and treatment services such as Vaccinations and Well Child Care, WIC Program, Family Planning, Evaluation and Treatment of Sexually Transmitted Infections, Rabies Prevention Services, Environmental Health, and Community Health.

Offers primary medical care to adults, children and teens as well as diabetic care and reproductive health care. Provide evaluations, give education, administer immunizations, draw samples for laboratory testing, determine diagnosis and treatment, and make referrals to outside specialists.

Provides coverage and reimbursement support, patient assistance and information resources.

CrossOver specializes in the treatment of chronic diseases, providing comprehensive treatment plans for patients suffering from diabetes, hypertension, chronic obstructive pulmonary disease, cardiovascular disease, and other chronic conditions through the Chronic Disease Management Program. Through a partnership with the VCU School of Pharmacy, also offer intensive diabetic management through the ambulatory care clinical pharmacist (PharmD) program. CrossOver provides a medical home to our patients, offering an innovative integration of health services under one roof, including primary and specialty care, chronic disease management, medication and medications management, dental, vision, mental health counseling, OB/GYN, HIV diagnosis and treatment, social work case management, and health education.

Offers care for chronic conditions by appointment only. Conditions include Diabetes, Asthma, GERD, Hypertension, and other chronic diseases.

A respiratory specialty medical clinic offering: * Screening and diagnosis. * Preventive care for respiratory infections. * Medical treatment. * Medical physicals. * Laboratory services. * Patient education. * Pulmonary rehabilitation. * Spirometry. * Smoking cessation. * Audiometry. * Oximetrey. * Benefits counseling and lay advocacy support, a benefit which extends to widows of miners as well. Stone Mountain Health Services Benefits Counseling and Lay Advocacy Component have proven to be and are still one of the leading resources in the nation in securing Federal Black Lung Benefits for coal miners and their widows. The Black Lung Staff continues to work collaboratively with each patient's primary health care provider to ensure that medical treatment is ongoing and meeting the patient's needs. Those patients without a primary care physician will be referred to a physician of their choice or a Stone Mountain Health Services Provider.

Provides Social Security/Supplemental Security Income Benefits, processes claims for approval or denial, maintains beneficiary rolls, earnings records maintenance, and issues Social Security numbers/cards. Processes Medicare applications and answers questions regarding Medicare.

Healthcare and benefits assistance for recently discharged veterans with service in a theater of combat operations.

Assists area residents in emergency situations for such items as heat, electric, and rent. May also provide gasoline assistance when needed to get to doctors appointments.

Provides hospital and medical insurance for people aged sixty-five and older; and for disabled individuals younger than sixty-five.

This independent division of Patient Advocate Foundation provides small grants to patients who meet financial and medical criteria. Grants are provided on a first-come, first-served basis and qualifications for each fund may differ based on fund requirements. Comprehensive medical services including: treatment for hypertension, diabetes management, women's health services including pregnancy screening, pediatric services, work and school physicals, laboratory and x-ray services, flu and pneumonia vaccines, treatment for minor illnesses, prescriptions for medicine, and referrals to specialty care practitioners. Also offers preventive care, annual check ups, diabetes and blood pressure checks, and screenings for sexually transmitted diseases and HIV.

Provides a crisis intervention program that offers food, clothing, housing assistance (to avoid foreclosure or eviction), fuel assistance, transportation or medical emergency. Clients applying for cash assistance must be able to pay a portion of the bill. Clients are eligible for cash assistance only once a year. Gives referrals for services not available directly.

Offers a Service club to assist the visually impaired and hearing impaired with assistive devices, such as glasses and hearing aids. Must be able to demonstrate financial need for assistance in obtaining necessary eye examinations and/or glasses.
