Identifies persons at risk for sensory loss/changes from diabetes and refers those at risk for appropriate medical care. Screenings are held at Health Fairs and at the Rockingham County Fair.

The American Kidney Fund works to raise awareness of kidney disease among people who are most at risk and the general public. MIKE stands for the Minority Intervention and Kidney Education program. This program's mission is to reduce the high rate of kidney failure in minority communities. The American Kidney Fund's MIKE program partners with public and private organizations, including government agencies, community and faith based organizations to provide education, screenings and follow-up services for people who are most at risk for kidney disease. Outreach Initiatives - The American Kidney Fund works with local community partners in various settings ( workplace, faith-based, civic groups, business, salons etc.) to raise awareness of kidney disease and risk factors through educational workshops and screening activities initiatives. American Kidney Fund Screenings - The American Kidney Fund works with partners nationwide to provide free educational materials and kidney health screenings in selected city venues. Diabetes Screening
Provides programs for senior citizens. Provides nutritious meals to seniors, transportation is provided. Seniors enjoy fellowship, informational health programs, crafts and recreation. Shopping outings and other social activities may be arranged. Health care screenings are offered occasionally including: blood pressure screening and diabetes checks. Programs offered may be in collaboration with the District III Governmental Cooperative, the Mount Rogers Health District and other area agencies.

Provides a program designed in accordance with the American Diabetes Association clinical practice recommendations. Its purpose is to increase access to education, reduce medical costs and prevent complications for women with gestational diabetes. Initially, each participant meets with a diabetes educator for a gestational class, sets personal goals and receives follow up education in the OB Clinic.

Staffed by Nurse Practitioners, Public Health Nurses, and Health Educators, the mobile medical unit provides onsite health screenings and fairs to people in our local health department offices, schools, work sites, and other community locations. Health screenings available include: Blood Pressure; Body Composition Analysis/BMI/Height/Weight; Cardiovascular Risk Assessment (for cholesterol), Diabetes/Blood Sugar; Pulmonary Function Testing; Respiratory Fitness Testing; Employee Physicals; Osteoporosis/Bone Density; Hemoccult Testing; Assessment for Skin Cancer; Prostate Cancer/PSA; PAP Smear/Breast Exam; STD testing and treatment; and Adult Immunizations. Health education programs are also available and arranged to meet specific needs identified during the screening process.

Provides a program designed in accordance with the American Diabetes Association clinical practice recommendations. Its purpose is to increase access to education, reduce medical costs and prevent complications for people with diabetes. Initially, each participant meets with a diabetes educator to learn the basics of the program, receives a baseline assessment and sets personal goals. Group classes are available. Topics covered include nutrition, exercise, stress reduction and grocery shopping.
Nurse visits in rural senior centers and homebound settings in the Counties of Albemarle, Greene, Fluvanna, Louisa, Nelson and the City of Charlottesville. The health services team performs screenings for blood pressure, glucose levels, height, weight, vision, and hearing. Provides education about nutrition, safety, medications, mobility, hygiene, foot care, and exercise. Helps to treat infections and minor injuries. They also help facilitate coordination with participants' doctors and caregivers if requested by patient.

The Diabetes Management education program is recognized by the American Diabetes Association offering education on a variety of topics including Type 1 and Type 2 diabetes, medical nutrition therapy, insulin pump initiation and support, gestational diabetes and pre-diabetes. Includes assessment with a Certified Diabetes Educator for a medical history, a brief physical exam, and goal setting for behavior changes. Follow-up care with a dietitian or RN for weight management, blood glucose control, and progress toward goals. Private instruction with an RN for insulin initiation, adjustments and management. Nutrition Counseling includes meal planning, carbohydrate counting, weight management, and incorporating meals and exercise. Day and evening classes are offered.
