Foster teens who are at least 16 years of age. This program varies from year to year depending on the needs of the teens and the funding available for the State. Based on needs identified through an Independent Living Needs Assessment, the teens receive counseling and participate in group activities regarding subjects that will assist them in becoming competent adults and self-sufficient, productive citizens.

Provides technical assistance, oversight, and compliance to Centers for Independent Living (CILs). CILs provide services and advocacy to promote the leadership, independence, and productivity of people with disabilities. CILs work with both individuals, as well as the local communities, to remove barriers to independence and ensuring equality of persons with disabilities. Non-profit, community based non-residential organizations that are run by and for people with disabilities. Services include I & R, Advocacy, Independent Living Skills Training, Peer Counseling, Nursing Home and Youth Transition Services. Equipment for hearing impaired such as telephones, doorbells, pocket talkers, etc. Assistance for obtaining eyeglasses.

Horizon Center is a Psychosocial Rehabilitation Program (PSR) and day treatment program providing rehabilitative services for individuals with mental health and/or developmental disability concerns. Horizon PSR program provides an environment where individuals can develop or regain interpersonal and daily living skills, develop a supportive system of peers and providers, and benefit from a supportive structure.

The purpose of the Kenmore Club is to provide citizens who are living with the effects of mental illness the chance to work and become self-supporting members of the community. Members participate voluntarily in work units of their choice which focus on the duties necessary for the daily operation of the Club. Work units include: Food; Membership; Clerical; and Maintenance. By taking part in the unit activities members gain confidence, build self-esteem, learn job-related skills, and work with others towards a common goal of recovery. Kenmore Club also provides educational training and transitional employment opportunities.

Turning Point provides a way to intervene rapidly after a first episode of psychosis, and to provide wraparound services for the young person with the goal of getting them re-engaged in the community and less dependent on a service system. The early intervention program helps the young people and their families understand and manage symptoms of mental illness and/or substance use disorder, while also building skills and supports that allow them to be successful in work, school, and life in general.

Provides in-home and community-based support services to individuals with mental illnesses by providing health and medication management and basic living skills instruction. Program objectives include increasing socialization and natural supports and identifying productive day activities. Individualized, person-centered service plans document individual goals. Counselors work with each individiual to reach their goals.

Offers residence and care for seniors age 60+ at the following levels; Independent Living (apartments), Assisted Living and Intensive Assisted Living.

Housing for seniors 62 years or older.

Provides planned combinations of individualized activities, supports, training, supervision and transportation to individuals with intellectual disabilities to improve their skills or maintain an optimal level of functioning. The program is designed to provide training and support in non-vocational areas such as self care, independent living, communication, socialization and motor skills.

Offers 103 independent single level attached homes and is part of the continuum of care offered at Warm Hearth Village retirement community. Residents pay a Life-lease fee at occupancy based on the neighborhood and floor plan selected. A minimum of 40 percent of the Life lease fee is returned to the resident or his or her estate. Features six floor plans measuring between 1080 and 1541 square feet. Each home has two bedrooms, two full baths, a living room and a dining room. Some have a den, garage, eat in kitchen and mud room. The monthly service fee paid covers water, sewer, trash collection, pest control, outside lighting of walkways, evening security, maintenance of the home's appliances, ground maintenance, and response to the individual emergency call systems. Residents have access to Village services such as dining, exercise room, therapies, transportation, planned activities, and receive priority admission to next levels of care.

Transformed Support Services offers the following Mental Health Support and group activities: - 24 hour accessibility - Crisis Intervention - Case management services - Accessing and coordinating community resources - Collaboration with other involved agencies - Individual and Family Counseling in the home and community - Educational tutoring and monitoring of day support program attendance, behavior, and performance - Substance abuse counseling and preventive education - Conflict resolution and anger management - Behavior Modification Program - Independent living skills training - Intensive parenting skills training ALL SERVICES COVERED BY MEDICAID

Offers independent living to persons 55 years or older. Offers services such as on-campus doctor, dentist, optometrist and pharmacy. Full service bank, convenience store and gift shop.

Work with individuals with disabilities to teach basic tasks that lead to independence such as doing laundry, cooking, cleaning, budgeting, resume formatting, and employment readiness.

Provides 40 units of section 8 single bedroom housing units specifically for the very low income elderly and the handicapped managed by Community Housing Partners, Inc.

A social rehabilitation center based on the clubhouse model where adults with mental health or emotional disabilities gain employment skills in food service, clerical duties, housekeeping and maintenance. Social activities are available.

Good Shepherd of Northern Virginia provide appropriate resources to prevent or address homelessness and teach life skills required to help individuals and families return to stability for long term success. The Six Steps to Self-Sufficiency program is an intensive case management approach to goal setting for long term achievement of stability for households.

These programs serve people who are recovering from mental illness by providing a safe and supportive environment that promotes independent living, social and interpersonal skills, and relapse prevention. Attending daily classes provides education about mental illness, substance use, and appropriate medication. Engaging in volunteering opportunities and engaging in social activities promotes empowerment, recovery, and competency. Services include: Face-to-face Service Specific Provider Intake (SSPI) that identifies treatment needs Development of Individualized Service Plans (ISP) which identify an individual's needs and goals in the area of mental health recovery and stability in the community Psycho-educational activities to teach about mental illness and appropriate medication to avoid complications and relapse Social skills training, peer support, and community resource development to promote empowerment, recovery, and competency Opportunities to learn and use independent living skills, and to enhance social and interpersonal skills Field trips to provide training in an integrated setting in order to increase an individual's understanding or ability to access community resources and meet goals outlined in the ISP

LACIL provides five core services: Peer Counseling, Independent Living Skills Training, Information and Referral, Systems and Individual Advocacy and Transition. In addition, service Facilitation for the consumer-directed DD Waiver and the CCC Plus Waiver.
