A center and community based program for people with intellectual disabilities that serves ages 12 years through adulthood. Provides supports, as identified in Individual Support Plans, to increase socialization, adaptive skills, communication, and more through the use of community resources (field trips, volunteer opportunities, etc) and center-based activities (arts and crafts, personal hygiene education, horticulture, etc.).

The Harmony Day Support Program provides structured day placement and training for individuals with intellectual or other developmental disabilities who may not be eligible for other training programs but who could benefit from social skills, pre-vocational and instrumental activities of daily living training. The goal of Harmony Day Support is to: Utilize community supports and services such as educational programs, contract work, recreational centers and public libraries. Develop social, recreational and leisure activity skills. Develop functional skills. Increase the individual's integration into the community. Serves low income residents of the City of Charlottesville by providing affordable housing for low income families including seniors and disabled individuals. Medicaid Waiver programming at Goodwill focuses on providing supports to individuals so they may achieve their personal goals and objectives. The programs operate in accordance with the Virginia Department of Behavioral Health and Developmental Services licensing regulations. Individuals receive services both in the Goodwill center and community based settings depending on their preference and interests. Discovery Services are Day Support programs at Goodwill. Through the programs, individuals will receive the support they need to acquire, retain, and improve their skills in areas that they have identified. A wide variety of opportunities, in both community and center based settings, for peer interactions, enhancement of social networks, and to build natural supports will lead to greater independence. Individuals enrolled in Discovery Services may participate in Group Day Support, Community Engagement, or Community Coaching services. Programing is offered at the Roanoke Jobs Campus, Radford Jobs Campus, and Rocky Mount Jobs Campus. Individuals living in Goodwill's 31 counties and 13 cities service area are eligible for services, however are responsible for their own transportation.

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.

Services are designed to meet the needs of clients experiencing difficulties in their lives due to emotional, behavioral, and mental health issues. Provide transitional support and stabilization services to clients returning to the community from a more restrictive environment. The goal is to provide each client with individualized services and training so they can function as independently as possible. Each client will learn to utilize appropriate community resources to maintain stability in the least restrictive community setting.

Housing for seniors 62 years or older

Provides services to help prevent and treat Intellectual Disabilities and to enhance the quality of life of individuals who experience these conditions in the Cities of Buena Vista and Lexington and the Counties of Rockbridge and Bath, Virginia by providing individualized treatments, establishing and expanding programs and support systems, and promoting integration into the community. Offers group homes and adult day activities programs to enhance the quality of life for adults with intellectual disabilities by providing comprehensive support services.

Residential Services take place in a licensed or DBHDS-authorized residential home. These services consist of skill-building, routine supports and health and safety support; this enables the person(s) supported to acquire, retain, or enhance their self-development, socialization, and adaptive skills necessary to reside successfully in a community-based setting – natural home environment.

A day support program, which enhances independent living skills and maximizes opportunities for community integration. Activities include life skills groups, support groups, recreation and socialization opportunities. The supported employment program provides employment services designed to teach the basic work habits necessary to be successful in obtaining and maintaining employment.

A residential facility for adults with intellectual and developmental disabilities that offers programs specifically designed to help them live as independently as possible. Training includes daily living skills, support for community living, ongoing support services.

Offers studio, one and two bedroom apartments with a variety of floor plans. The Wellness Center at Richfield offers residents living in our apartments a place to achieve their wellness goals. Offering a gym equipped with exercise equipment, yoga/exercise classes, individual wellness coaching, outpatient therapy and a salt water therapy pool. Access to a full continuum of health care services.

Designed to help people with disabilities in learning new independent living abilities. The staff works directly with the consumer to teach these new skills through hands-on instruction. Together they write an independent living plan to meet individual needs, such as self-esteem, meal planning, financial management, obtaining housing and transportation and socialization. Clear Brook Apartments is a complex for lower income seniors that are 62 years of age and older. Rent is based on income.

The center provides a comprehensive range of services tailored to the specific needs of the community, including food, clothing, addiction recovery support, computer training, educational programs, health clinics, job training/placement, legal aid, ESL classes, life skills, and parenting classes.

The purpose of LIFT is to provide apartment-based Independent Living and Support Services to help foster care youth, ages 17-21, transition from residential care to independent living. Youth receive housing, vocational and educational services, life skills training, therapeutic services (counseling, therapy and/or substance abuse education), and the development of a network of support services in the community. LIFT is licensed for 25-beds for males & females transitioning from the foster care system to independent living in the community.

Meadowcreek Day Activity provides training and assistance to adults with developmental disabilities to acquire, improve and maintain functional abilities, while also achieving social integration with peers and the community. Meadowcreek Day Support provides opportunities for individuals to pursue personal interests and strengths in the community through art, music, technology, self-advocacy, and wellness. Meadowcreek coordinates with a variety of community-based services, including Charlottesville Parks & Recreation, Access Arts, Madison House, Meals On Wheels, Best Buddies, and Salvation Army.

BreakThru Services provides Mental Health Skill-Building services that offers goal-directed training to enable individuals with mental illness to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. BreakThru Services LLC will also provide training in the following areas to the individuals we serve: Use of community resources to include: Mental health resources Department of Social Services contact information How to utilize emergency services to include: food, rent, and financial assistance Contact information for transportation services, physicians, dentist, psychologist, counseling, food pantries, as well as medical doctors and case workers. We will assist in helping the individuals we serve locate these resources and collaborate with these professionals as needed (if the client gives permission in writing). Training in: Functional skills and appropriate behavior related to the individual's health and safety Activities of daily living Assistance with medication management Monitoring health Nutrition and physical condition

The Pozez JCC of Northern Virginia's Special Needs Program offers unique programming for individuals with special needs. The program is designed to develop physical and social skills by uniquely combining therapeutic sessions with inclusion programs. The program features small participant-staff ratios and offers social programs, family events and recreational social skills classes. During the pandemic, programs are primarily offered virtually, however, social skills classes are offered in-person with safety measures in place.

Provides day support services and a Lifeskills program for individuals with developmental/intellectual disabilities. Services are person-centered and community-based.
