Homelessness Prevention - North Tazewell, Clinch Valley Community Action

This program prevents the displacement of individuals and families from their homes as a result of eviction or foreclosure. A short-term rental assistance program offered to individuals and/or families who are in financial crisis or who are facing homelessness. This program is part of the Balance of State Continuum of Care. We work with our local planning group (LPG) in collaboration with People, Inc. Provide services in Tazewell County, and People, Inc. provides centralized intake and assessment for each of the participating localities.

Housing Homeless Intervention - Winchester, Supportive Housing Program, Northwestern Community Service Board

Provides outreach for homeless people who are disabled from substance abuse or aids and/or mental illness and homeless families with children. Supportive services program offers case management, triage, budgeting, referrals, medication, assistive technology, information and referral and other services. Also offers residential housing (rent assistance) for homeless persons with disabilities through the Shelter Plus Care (S+C). Homeless Prevention - Charlottesville, Central Intake, Haven, The

The Homeless Information Line (HIL) is the link to services for those who are homeless or are at risk of becoming homeless in the next 14 days. Callers leave a message and will receive a call back within 3 business days. The callback will review their circumstances, provide links to resources, and if they may be eligible for back rent or need assistance with a security deposit/first month rent to leave homelessness, they are referred for an eligibility screening, which determines if they can be referred for help from the Haven's housing programs. Those who are not eligible for Haven funds are referred to other resources in the region specific to their needs.

Transitional Housing - Arlington, Sullivan House, Bridges to Independence

Bridges operates an emergency family shelter, known as the Sullivan House, which has approximately 45 beds across 10 separate apartment units. The program provides shelter and case management services to homeless families leading to employment, permanent housing, and achieving self-sufficiency. It is a temporary solution for those with little or no resources. While there, families receive intensive case management - assessing their situation, providing Bridges program resources, or referring to other resources. The families receive whatever immediate needs they may have - food, diapers, personal supplies, basic kitchen items, gift cards and more. The length of stay generally is between 30-90 days, and then Bridges works with the families to move them into permanent solutions

Provides prevention and diversion counseling and can pay rental arrears or security deposit for persons who are not homeless but in danger of homelessness. Provides assistance to individuals and families struggling with an unexpected crisis or homelessness. Housing and budget counseling are major components of this assistance to help each client develop goals to work toward self-sufficiency. Provides financial assistance to clients experiencing some kind of short-term crisis beyond their control, such as a serious accident, illness or injury; an unexpected job layoff, termination or reduction in hours; a loss of daycare or transportation that has affected a client's ability to work; or a delay in approved benefits.
