Eligibility
The individual must meet all of the following:
- Diagnosis of a serious mental illness
- History of a psychiatric hospitalization or a similar intensive intervention
- Have had a prescription for psychotropic medications within the last year

A member should be referred for services, they can use our website, come to the office, fax in a referral, or call over the phone to complete. We will check eligibility during the referral process.
Fees
This service is reimbursed through a member's VA Medicaid (No cost for the member).

Assist individuals, over age 18, who are diagnosed with serious mental illnesses to live independently in the community. Focus for treatment is based on developing basic living skills in the following areas:
- Attending medical and psychiatric appointments
- Medication management
- Symptom management
- Money management
- Health and safety
- Personal hygiene
- Nutrition
- Food preparation
- Use of community resources
- Development of social skills
- Building a personal support system
- Activities of daily living
