Must bring: Picture ID for everyone 18 or older in the household; School records for everyone school age in the household; The last three (3) paystubs of every working person in the household; Proof of residence such as a lease, a current utility bill, a land line telephone bill, bank statement, or a letter from the school verifying your address (no more than 60 days old)
