Description

Vital Records provides certified copies of death certificates, applications for birth certificates, and applications for declaration of paternity. Certified copies of birth certificates for births that occur in the State of Virginia must be obtained from the Virginia Department of Health, Office of Vital Records and Health Statistics, 2001 Maywill Street, Richmond, VA 23230.
