To register your child please bring the following: 1)Child's original birth certificate 2)Child's Social Security number 3)Proof of county residency 4)School Entrance Health Form completed by a licensed medical provider. Part I-Health Information Form, Part II-Certification of Immunization, Part III-Comprehensive Physical Examination Report completed within the twelve months prior to the date the child first enters kindergarten**
