If you are uninsured, please fill out the Patient Application (availible online) and then call the Center to find out when you can meet with a Patient Services Representative to go over your paperwork. Once you have met with the representative, you can start making appointments. If you have insurance, please call the Center to schedule an appointment.

Serves uninsured individuals, as well as people who are insured with Medicaid, Medicare, and private insurance. Services provided are medical, GYN, dental, and mental health counseling services to people of all ages. CHCNRV offers a sliding-fee program to anyone who qualifies.
