Must be 18 years + and a resident of Virginia. Must have a physical disability. Considered a last resort when insurance, Medicare or Medicaid will not provide. If the request if for equipment valuing over $500.00, you are required to submit a "Letter of Medical Necessity" from your physician (a form is provided in this application).
