Care Connection for Children - Blue Ridge, Charlottesville, Virginia Department of Health

Blue Ridge Care Connection for Children serves children from birth to the age of twenty-one with special health care needs. Provides care coordination, including referral to health care services, case management and referrals to community support and resources. Maintains a database of regional and local resources for children with special health care needs. Financial assistance may be available to assist with specialty medical care,funds are available, if income and medical criteria are met. Coordination of care and services are provided by case managers - registered nurses, social workers, community resource coordinators, educational consultant, and parent resource coordinator. Assist families to participate in the planning and decision-making about their child's services and plan for the child's future. Our educational consultant support the child and families with educational needs and facilitates relationships with the child's school. Our Parent Resource Coordinator provides a network of experienced parents and other family caregivers of children with special health care needs that for the parent to parent support.
